Job Title:  TCU Admissions RN

Status: Non-Exempt    

 Coordinates the facilityâ??s referral, inquiry and admission process.  Provides all stakeholders with facility related information through facility tours, phone calls and clinical assessments.  Assists the LTC Admissions Coordinator and provides coverage making daily patient placement decisions, coordinating admissions process, and maximizing bed occupancy. Completes pre-admission evaluations. Responsible for quality service delivery and internal/external customer relations for Madonna as a whole, including upholding the mission and values for the department and facility.
*  Logs and responds to inquiry calls/referrals from hospital discharge planners,  
    physicians, insurance companies, families and other referral sources per facility
*  Coordinates and completes pre-admission evaluations, including assessing    
    patientâ??s rehabilitation potential, interviewing patient and/or family to
    determine any special needs, obtaining financial information, and conducting
    hospital visits as needed. 
*  Tracks, monitors and updates pre-admission screening information so that
    clinical information is current facilitating patient admission.  Monitors all active
    and inactive inquiries.
*  Makes decisions on patient placement based on pre-admission screening
    results, program admission criteria, and internal resources.
*  Assigns patients to inpatient programs, coordinates patient admissions when
    covering as Admission Coordinator.
*  Maintains TCU bed availability and program/service information to manage and
    maximize occupancy levels. 
*  Communicates admission acceptance to clinical liaisons/referral sources
    coordinating patient transfer to ensure a smooth transition to Madonna. 
*  Communicates admission denial to clinical liaisons/referral sources identifying
    alternative program placement options as indicated. 
*  Communicates daily admission activity to internal facility staff to ensure
    appropriate room and bed availability facilitating a smooth transition to
*  Arranges for essential equipment, supplies, and services pre-admission to
    ensure smooth patient transitions
*  Assists LTC Admission Coordinator to resolve any issues regarding daily patient
    transfers both pre and post admission.
*  Maintains a working knowledge of Federal and State regulations and
    reimbursement (Managed Care, Medicare and Medicaid). 
*  Maintains a working knowledge of the managed care referral process. 
*  Instructs and orients facility staff to regulations, facility programs, pre- 
    admission operational definitions as well as corporate and departmental mission
    and goals to ensure compliance and delivery of quality service to all internal
    and external customers. 
*  Conducts routine meetings with facility staff to ensure staff is updated timely
    and obtains staff input and feedback on prospective admissions. 
*  Encourages the promotion of Madonna by effective communication skills with
*  Follows instructions from supervisor to perform other functions as assigned in  
    order to achieve corporate and system goals. 
Indoor - Sedentary work - Occasionally lifts and carries 30 pounds when stocking supplies, and/or transporting reports or equipment. Some climbing and balancing to reach computer books and supplies stored on high shelves. Some stooping and crouching to do filing.  Frequent reaching and handling of various papers. Ordinary talking and hearing to answer inquiries by telephone or in person continuously. Seeing acuity, near and far continuously, with depth perception. Mobility required for travel between campuses and to scheduled events or meetings as needed.
QUALIFICATIONS (Education/training and/or Experience)
Must possess a current Nebraska R.N. licensure and three years applicable work experience. Must possess excellent communication skills, both written and verbal, and have demonstrated ability to work independently and prioritize responsibilities.  Valid Nebraska driverâ??s license and proof of good driving record required.
