The Behavioral Health Coordinator is a professional person with the overall responsibility for directing the programmatic and clinical aspects of the day and residential program. Significant aspects of the work include: casework with individuals served by the program to address mental health or behavioral health needs; being familiar with a variety of therapeutic models including Applied Behavior Analysis, consulting with and training Program Managers and Direct Support Professionals in behavioral interventions including documentation and behavioral support strategies, oversight of the Individual Program Plan (IPP) process, developing and maintaining effective partnerships with local and state agencies, developing and maintaining interagency and community relationships, and exercise of independent judgment within a broad framework of rules, regulations and policies to insure compliance with DD regulations.

Responsibilities Include:

Program Development

  • Participate in the screening of all prospective consumers by reviewing referral packets, interviewing the individual, and meeting with involved family, clinicians and service coordinators
  • Make recommendations of needed supports and environmental adaptations
  • Reviews case management activities, goals and rules with Rejoice DDS team

Behavior Support Plans

  • Completes Functional Behavior Assessments which helps to provide direction for all services.
  • Developing, training and implementing Behavior Support Plans (BSPs), utilizing positive behavior supports including: behavior modification strategies, community integration, health, recreation, activities of daily living, etc. to ensure quality service delivery
  • Develop and train Safety Plans for individuals served.
  • Write and Review of data collection on BSPs, FBA on at least a monthly basis
  • Responsible for supervising, coordinating, and monitoring the delivery of treatment and clinical services and ensuring prompt and proper implementation of all clinical aspects of ISP mandated services
  • Provide ongoing consultation and when necessary supervision to Managers and Direct Support Professionals in all aspects of the IPP system and the delivery of high quality services to the Individuals served by the residential and day program
  • Attend trainings and workshops as needed to stay current with the ever evolving clinical service delivery
  • Coordinate materials for the Internal Review Committee (Human Rights), Department of Health and Human Services, Developmental Disability Systems, and peer review as needed.
  • Maintain BSP goals database
  • Review General Event Reports, T-logs and data collection on Therap to make recommendations regarding effectiveness of behavior support plans, safety plans, etc.

General Duties

  • Collaborate with the management team to ensure comprehensive training programs that will equip all staff to perform all programmatic duties competently.
  • Attend ISP meetings for clients with behavioral needs (2x per year for each individual)
  • Liaison to families, service coordinators, medical personnel, consultants, providers, and Internal Review Committee
  • Monitor incident reports
  • Develop and maintain working relationships with other agencies, community organizations, or other interested parties regarding program objectives and services
  • Collaborate with the Quality Assurance Director to insure compliance with all licensing, regulatory and/or contractual obligations
  • Conduct periodic on-site review of service delivery
  • Prepare various documents and reports as required or requested by agency and DDS officials
  • All other duties as assigned.

Qualifications

  • This position must be held by a licensed individual. Minimum of Licensed Mental Health Practitioner (LMHP) is required, Independent Practitioner’s License (LIMHP) is preferred.
  • 3-5 years case management experience in either DD/mental health
  • 3-5 years experience working with individuals suffering from mental health diagnosis
  • Experience developing and managing treatment plans for mental health individuals
  • Ability to take lead role in discussion of individual mental health/crisis/intervention strategies
  • Develop and maintain working relationships with other agencies, community organizations, or other interested parties regarding program objectives and services
  • Outstanding interpersonal, verbal and written communication
  • Outstanding attention to detail
  • Ability to manage multiple priorities.


Summary: The Behavioral Health Coordinator is responsible for directing the programmatic and clinical aspects of a day and residential program for individuals with mental health or behavioral health needs. This includes casework, training and consulting with staff, overseeing the Individual Program Plan process, and developing partnerships with local and state agencies. The ideal candidate will have a minimum of 3-5 years of experience in case management and working with individuals with mental health diagnoses, as well as a LMHP or LIMHP license.
