SUMMARY DESCRIPTION OF WORK

Entry-level professional positions begin at this level. Provide home and community based family services, including crisis intervention, counseling, skill building and advocacy. This is a 24 hours a day, 7 days a week program. 

Compensation: between $16.83 and $20.06 per hour (wage is determined upon number of years of experience)
Work Schedule: as needed (will vary)

MINIMUM QUALIFICATIONS

  • Bachelor’s Degree in social work or related field required
  • Experience with in-home services preferred
  • Valid driver’s license, acceptable driving record

Essential Duties and Responsibilities

  • *Must be able to maintain a flexible, accessible, and responsive work schedule.
  • Carry team back – on-call phone - as assigned and provide coverage/ back up to supervisor as needed.
  • *Schedule and complete client intake according to program contract or requirements.
  • *Conduct holistic, strength-focused assessment process.
  • *Provide a range of clinical, concrete and advocacy services to family members that are consistent with the family’s values, learning styles, lifestyle, circumstances, and culture.
  • *Be aware of and respond appropriately to potential safety issues.
  • *Provide in-home and on-site education and skill building activities.
  • Provide culturally and ethnically appropriate family services and intervention strategies.
  • *Utilize motivational enhancement and cognitive/behavioral strategies.
  • *Utilize a collaborative goal setting and teaching approach.
  • *Use interventions with clients that are theoretically based.
  • *Use effective strategies for engaging, defusing and confronting clients through collaborative problem-solving techniques.
  • *Assess ongoing support and service needs.
  • *Develop and facilitate linkages for families with community resources and informal supports.
  • *Provide ongoing evaluation and modification of family goals and service plans.
  • Assess goal progress and family needs and resources.
  • Solicit feedback and input from the family about all aspects of the intervention.
  • Implement discharge process at end of intervention.
  • *Is dependable and punctual regarding scheduling and attendance.
  • Is prompt and accurate in completing all required reports, statistics, and other requirements.
  • Displays a courteous and caring attitude at all times to the clientele, volunteers, and visitors of the Agency.
  • Cooperates and collaborates with program area staff, volunteers, and other Agency staff.
  • Abides by all specific program and Agency procedures, policies, and requirements.
  • Develops personal and program related skills through participation in internal or external training opportunities including printed material and audio and/or video media.
  • *Creates, maintains and shares as appropriate a dynamic self-care plan.
  • *Strives to make connections between the agency and the larger community whenever possible in order to contribute to the agency’s ongoing fundraising and friend-raising efforts.
  • *Essential functions of this job are to be performed on company physical work site.
  • Performs other program related duties as assigned.
