Internal Only Lead provision of patient-oriented pharmaceutical care services including creation and maintenance of pharmaceutical care plan, cost-effective therapeutic drug monitoring, drug utilization, guideline development, and drug use evaluation in assigned clinical teams. Provide training and educational support for development of clinical skills in professional staff. Work proactively as a member of a team to ensure the provision of optimal pharmaceutical care. Demonstrate a dedication to continual and constant quality improvement.
***Monday through Friday, No Weekends and No Holidays. Support of our Bone Marrow Transplant Services***
Minimum of five years clinical practice experience in the clinical specialty or an American Society of Health-System Pharmacists (ASHP) accredited postgraduate year two residency in the specialty required.
Graduate of an Accreditation Council for Pharmacy Education (ACPE) accredited Doctor of Pharmacy degree program OR possess a Bachelor or Master of pharmacy degree with 10 years of experience required.
Licensed in the state of Nebraska or jurisdiction in which pharmacy practice is conducted upon start or transfer required. Basic Life Support (BLS) certification as deemed appropriate and relevant to practice required.
American Society of Health-System Pharmacists (ASHP) accredited postgraduate year one or year two residency as deemed appropriate and relevant to practice preferred.
Board of Pharmaceutical Specialties (BPS) certification as deemed appropriate and relevant to practice preferred.
