Description

  • Purpose of Job
    • The Physical Therapist HH (Home Health) restores patient's function, alleviates pain, and prevents disabilities by planning and administering medically prescribed physical therapy. In the home health care setting, the Physical Therapist functions as Case Manager, using the development of an interdisciplinary plan of care and manages resources to achieve identified outcomes.
  • Job Requirements
    • Education
      • Successful completion of a Physical Therapy bachelor’s degree, master’s degree or doctorate degree from a Commission on Accreditation in Physical Therapy Education (CAPTE) accredited program required.

    • License/Certifications
      • Unrestricted Nebraska Physical Therapy License required.
      • BLS (American Heart Association - Basic Life Support) certification required.
      • Requires the ability to provide transportation to other campus and/or work related locations within a reasonable timeframe.

    • Experience
      • 1 year Physical Therapy experience preferred.
      • Home Care experience strongly preferred.
      • Electronic Medical Record entry experience preferred.

    • Skills
      • Requires knowledge as demonstrated by successful completion of an accredited program.
      • Requires skills in communication, both oral and written, time management and organization, and personnel management.
      • Requires the ability to work independently, adapt to a changing environment, organize and work with a variable patient load.
      • Requires the ability to conduct patient teaching and in-service presentations.
      • Practices within standards of practice as outlines by professional organization and licensing body.
      • Requires ability to properly document patient visits within a reasonable amount of time following the patient visit.
      • Ability to travel to client's homes to provide service.
      • Requires ability to effectively evaluate patients and functional assessments, as well as apply and/or direct appropriate treatment.
      • Ability to communicate/coordinate care and findings with referring providers.
  • Physical Requirements
    • Physical
      • Frequently stand/walk.
      • Occasional Stair Climbing. Frequently lift/handle/carry moderate weight (20 to 50 lbs.).
      • Frequent hand use.
        Frequent grasping, forceful gripping, pinching.
      • Ability to safely operate mechanical lifts and transfer devices.
      • Ability to manipulate various tools requiring adequate eye/hand and fine motor coordination.
      • Occasional squatting and stooping.

    • Environmental
      • In inpatient and outpatient settings, indoor environment with frequent exposure to mild discomfort from dust, fumes, temperature, and noise.
      • In home health care settings, frequent exposure to extreme conditions, temperature, dust, fumes, pets, vermin, and other environmental factors in patient’s home.

    • Hazards
      • Occupational Safety and Health Administration (OSHA) Category 1: Tasks that involve exposure to blood, body fluids, tissues, and other potentially infectious materials.
  • Essential Job Functions
    • Essential Functions I
       

      • Provides quality care consistent with the State of Nebraska Statutes Relating to Physical Therapy Practice Act by assessing and interpreting evaluations and test results to determine physical therapy treatment plans in consultation with physicians or by prescription to meet the goals and needs of the patient.
        • Documents patient care services by charting in patient records.



           

      • Helps patient accomplish treatment plan and accept therapeutic devices by instructing, encouraging, and assisting patients in performing physical activities, such as therapeutic exercises, ambulatory functional activities, and daily-living activities and in using assistive and supportive devices, such as crutches, canes, and prostheses; evaluating patient's progress and recommending adjustments and modifications; directs technicians/Home Health Aides and assistants.

      • Actively participates in a multidisciplinary team providing patient-focused care, assessing and evaluating patient progress toward achievement outcomes and effectiveness of care, revises plan appropriately upon consult with patient’s physician.

      • Completes discharge planning by consulting with physicians, nurses, social workers, and other health care workers; contributing to patient care conferences and assures continuation of therapeutic plan following discharge by designing home exercise programs; instructing patients, families, and caregivers in home exercise programs, disease process, self-care techniques, prevention strategies, and assuming responsibility for providing physical therapy care; recommending and/or providing assistive equipment; for home health care or inpatient, recommending outpatient services or for inpatient, recommended home health follow-up programs.

      • Manages assigned caseload, assesses client needs and develops an interdisciplinary plan of care with client/family participation.
        • Organizes and coordinates care with appropriate referral to specialty teams and internal/external resources.




           

      • Maintains patient confidence and protects hospital operations by keeping information confidential.

      • Maintains safe and clean working environment by complying with procedures, rules, and regulations.

      • Protects patients and employees by adhering to infection-control policies and protocols.

      • Enhances physical therapy practice and organizational reputation by accepting ownership for accomplishing new and different requests; exploring opportunities to add value to job accomplishments.

    • Essential Functions II
       

      • Participates in mandatory in-services and/or CE programs as mandated by policies and procedures/external agencies and as directed by management.

      • Follows and understands the mission, vision, core values, Living our Values and company policies/procedures.

      • Other duties as assigned.
