Job Description

  • Purpose of Job
    • Accountable for the delivery of quality care to patients and families according to the individualized needs of the patient.
    • Provides diabetes expert care in the outpatient and inpatient settings.
  • Job Requirements
    • Education
      • Bachelors of Science in Nursing (BSN) from an accredited school of nursing required.
      • Successful completion of annual mandatory education requirements.

    • Experience
      • Three years clinical nursing experience with progressively more responsibility with clinically challenging patients.
      • Minimum of 2 years diabetes education experience; CDE or CDE eligible.

    • License/Certifications
      • Current valid Registered Nurse (RN) License, valid compact multi-state license, or a temporary permit while awaiting licensure required for work state.
      • Basic Life Support (BLS) Certification required.
      • Certification(s) in area of specialty preferred.
      • Certified Diabetes Educator (CDE) preferred.

      • Strong interpersonal skills necessary to effectively communicate and interact with patients, family members, hospital personnel, and co-workers.

      • Provides appropriate care specific to the age of the customer/patient, to ensure understanding and comfort level of treatment as outlined in the Population Specific Criteria.
        • Demonstrates ability to identify opportunities for and give individualized customer service/patient care according to customer/patient needs.

      • Performs an initial and ongoing assessment to determine the patient's needs.
        • Includes data from the patient/significant other(s) in the assessment as appropriate.

        • Communicates to appropriate personnel regarding unresolved issues.
        • Works within the scope of practice as defined by Nurse Practice Act.

        • Charges accurately for use of supplies and equipment according to policy.

      • Maintains knowledge of current diabetes practice as per American Diabetes Association diabetes self-management education program recognition guidelines.

      • Ensures quality education will be delivered to patients with diabetes who require education.
        • Provides individualized diabetes education based on identified patient needs and the nursing process.

      • Effectively collaborates with others to provide quality care to the patient with diabetes.
        • Works with staff nurses and food service staff to assess, plan, implement and evaluate patient education of diabetes.
        • Collaborates with physicians and other health team members to provide quality care.

      • Participates in the operations of the Center For Diabetes & Nutritional Health to:
        • Meet the needs of the hospital for diabetes services.
        • Promote the philosophy and marketing plan.

      • Markets the Center to Administration, physicians, hospital personnel, and community.

      • Acts as consultant/resource in all aspects of diabetes care to hospital and community.
        • Provides learning opportunities for health care professionals and students regarding diabetes care and education.
        • Communicates with other programs and providers to ensure effective use of Health System and community resources.

      • Participates in the development and implementing the performance improvement action plan.

      • Use interpersonal techniques with individuals that are intended to:
        • Result in desired objectives/outcomes.
        • Be perceived as satisfying/acceptable by those involved.
        • Demonstrates a team effort with both employees and physicians.
        • Handling customer relations with patients and families including complaints, concerns, and compliments.

    • Essential Functions II

      • Participates in mandatory in-services and/or CE programs as mandated by policies and procedures/external agencies and as directed by management.
