Job Description

  • Work Monday - Friday 8:00AM - 4:30PM and work every 3rd weekend 8:00AM - 4:30PM with two days off during the week. Work one holiday per year.

  • Purpose of Job
    • Responsible for the management of the initiation, planning, execution, monitoring and control phases of the community care coordination project; assistance in the administration functions of the Director of Case Management; and budget and tracking for the Foundation.

  • Job Requirements
    • Education
      • Bachelors degree in computer related field and/or 3-5 years direct Project Management experience required.

    • Experience
      • 3 to 5 years of clinical experience working in Project Management.

    • License/Certifications
      • Project Management Professional (PMP) certification strongly preferred.

    • Skills/Knowledge/Abilities
      • Accomplishes project objectives by planning and evaluating project activities.
      • Knowledge in planning, delegation, time-management, and problem-solving required.
      • Strong Excel, Access and Microsoft Word skills required.
      • Knowledge of Tav Health and Cerner and advantage.
      • Social media skills required.
      • Strong data analysis, tracking and reporting skills required.
      • Requires excellent oral/written communication skills to give formal presentations, provide excellent leadership to staff, and also the ability to execute project management skills.
      • Ability to make accurate and effective decisions at the project level.
      • Flexible, adaptable to change and able to work in a fast-paced environment requiring multi-tasking.

        • Creates a work plan for community care coordination project.
        • Reviews the work plan with Director and revises as required.
        • Manages day to day operational aspects of the project.
        • Provides updates to Director and project team.
        • Manages project expectations with Director and project team.
        • Assists in the development and sustainability of the project.
        • Develops, manages and monitors the TAV Health system.
        • Trains TAV Health users for 20+ organizations.
        • Compiles, reviews and presents project reports.

      • Project Accounting - Identify, manage and minimize project risks.
        • Identifies, assesses and minimizes project risks until successful project completion.
        • Manages numerous project budgets and risks associated with the projects.
        • Directs and coordinates tasks of the project to ensure project progress stays on schedule and within budget.
        • Manages, coordinates and tracks a voucher system for the project.
        • Tracks project deliverables for project expenditures and report actual cash flow and variances to Director monthly.
        • Ensure that all financial records for the project are up to date.

      • Project Coordinator - Works with internal and external customers to understand and execute the logistics of the project including ED utilization, medical home providers, community health care workers, insurance payers and behavioral health needs of the community.
        • Interacts with internal customers and outside vendors regarding services that are provided.
        • Maintains a level of expertise and current knowledge base on available community resources and services.
        • Coordinates meetings and planning sessions with internal and external vendors.
        • Coordinates and attends internal and external meetings to ensure oversight and compliance of implementation and maintenance of program.
        • Manages internal customers and outside vendors by mentoring, coaching, training, supporting and establishing objectives to promote successes and growth for the project.
        • Serves as resource for internal and external customers for questions regarding project.
        • Communicates effectively with partners to identify needs and evaluate alternative solutions.

      • Program Management - Establish and maintain project files to include planning, task time frames, records of meetings/attendees, project progress, costs and other related project communications.
        • Works independently to plan, recommend and implement tasks.
        • Maintains record of attendance and minutes of required meetings.
        • Maintains project documentation.
        • Establishes and maintains a well-communicated schedule for all project tasks.
        • Develops and maintains the project website and facebook page.

      • Research & Practice Development - Executes project software and ensure that all project information is appropriately documented.
        • Understands the principles of Tav Connect as wells as process and procedures for entering data.
        • Develops, updates and maintains forms needed to document project activities and manage data (assessment, consent forms, etc.)
        • Monitors data entered for timeliness and accuracy and assures that data is reliable and valid measures are obtained.
        • Educates and explains the project software and process of entering data to internal and external customers.
        • Provides ongoing education based on needs of the project and serves as a resource for questions regarding the project software.
        • Assists in using the data collected to identify opportunities for improvement of care coordination.
        • Participates in Performance Improvement process.
        • Participates in new employee orientation.
        • Willingly performs other duties as assigned.

      • Case Management - Assist Case Management Director with advanced data analysis, monitoring, and reporting for department.
        • Extracts data from Cerner and Tav to create and develop appropriate spreadsheets and report the results.
        • Data analysis for Community Care Alliance programs and organizations, linking work, insurance, barriers, medication, vouchers and all necessary demographics to ensure proper care.

      • Grant Budgeting/Applications - Research new grants available and support the Foundation with databases and budgeting of funds.
        • Researches new grants available.
        • Foundation support with databases and budgeting of miscellaneous grants.

    • Essential Functions II

      • Participates in mandatory in-services and/or CE programs as mandated by policies and procedures/external agencies and as directed by management.

      • Follows and understands the mission, vision, core values, Employee Standards of Compassionate Care/AIDET and company policies/procedures.
