  • Purpose of Job
    • Accountable for the delivery of quality care to patients and families according to the individualized needs of the patient.
  • Job Requirements
    • Education
      • Successful completion of annual mandatory education requirements.
      • Bachelors of Science in Nursing (BSN) from an accredited school of nursing required.

    • Experience
      • One year of Home Health experience preferred.

    • License/Certifications
      • Current valid license, valid compact multi-state license, or a temporary permit while awaiting licensure required for work state.
        • Registered Nurse
      • Must maintain current American Heart Association Basic Life Support (BLS) Certification.

    • Skills/Knowledge/Abilities
      • Strong interpersonal skills necessary to effectively communicate and interact with patients, family members, hospital personnel, and co-workers.
      • Ability to multi-task and prioritize work assignments.
      • Ability to learn, retain, and utilize computer skills that are required for department and hospital information system.
      • Ability to think critically and problem solve.

      • Provides appropriate care specific to the age of the patient to ensure understanding and comfort level of treatment as outlined in the age-specific criteria by:
        • Demonstrating age appropriate care for all patients.

      • Initiates decision-making and problem-solving related to assigned activities to assure quality patient care by:
        • Assessing appropriateness of current plan of care.
        • Implementing and revising the plan of care.
        • Performing technical skills competently and independently.
        • Evaluating progress toward goals
        • Meeting productivity standards set by organization

      • Participates in patient care delivery to achieve satisfactory patient outcomes by:
        • Adhering to Home Health policies and procedures.
        • Contributing to interdisciplinary care conferences.
        • Supervising care provided by other staff as assigned.

      • Contributes to operational and clinical records to comply with professional standards of practice, Federal and State regulations, and established policies and procedures by:
        • Assisting in record management as assigned.
        • Documenting all visit related items while in the home.
        • Completing accurate record documentation.
        • Meeting established time requirements.

      • Participates in Home Health Performance Improvement Program to ensure continuous improvement by:
        • Serving on Performance Improvement committees as requested.

      • Maintains expected department productivity standards to ensure agency financial stability by:
        • Verbalizing a basic understanding of insurance coverage issues.
        • Practicing cost effective care delivery.

      • Participates in on-call schedule to assure 24-hour RN availability to patients by:
        • Assisting in staff agency on-call needs.

      • Assists with staff orientation/development to ensure competency of staff by:
        • Acting as preceptor/mentor as requested.

      • Initiates action in response to information indicating risk to correct, prevent and/or minimize risk to the employee, agency, hospital and customer by:
        • Maintaining knowledge of and complying with department and regulatory policies and procedures.

    • Essential Functions II

      • Participates in mandatory in-services and/or CE programs as mandated by policies and procedures/external agencies and as directed by management.

      • Follows and understands the mission, vision, core values, Employee Standards of Behavior and company policies/procedures.

      • Other duties as assigned.
