Job Description

  • Purpose of Job
    • The NHN Clinical Assistant assists the Nebraska Health Network (NHN) in managing the contracted lives through reviewing medical records and claims files and assisting Population Health Coordinators with closing gaps in care.
  • Job Requirements
    • Education
      • High School Diploma or General Educational Development (G.E.D.) required.
      • Post High School education in a healthcare field required.
      • Licensed Practical Nurse (LPN) or Certified Medical Assistant (CMA) preferred.

    • Experience
      • Minimum of 3-5 years experience in a healthcare environment required.
      • Minimum of 1-3 years experience conducting medical records review required.
      • Minimum of 1-3 years experience processing claims or providing utilization management preferred.

    • License/Certifications
      • Valid certification or licensure in the State of Nebraska for area of competency required.

    • Skills/Knowledge/Abilities
      • Attention to detail.
      • Effective communication skills, both verbal and written.
      • Skilled in team participation and work coordination.
      • Ability to determine client teaching needs and preferred learning style.
      • Knowledge of medical terminology and procedures.
      • Knowledge of medicine including recognizing and diagnosed diseases, deformities, and other illnesses.
      • Ability to effectively manage time and prioritize tasks.
      • Ability to proficiently type information into the computer to capture or detail information quickly.
      • Ability to provide caring services when talking with patients and their family members.
      • Ability to critically listen to questions from patients and information from providers to convey information effectively.
      • Ability to problem solve during daily activities which includes the ability to be flexible.
      • Ability to learn and proficiently navigate through computer programs that contain patient information.
      • Able to follow written and verbal instructions/directions.

  • Essential Job Functions
    • Essential Functions I

      • Assists Population Health Coordinators with gap closure for members.
        • Provides patient outreach for specific patient populations with chronic illnesses such as diabetes, cardiovascular disease, hypertension.
        • Informs patient of community services which support current diagnoses.
        • Assists patient in scheduling appointments, sends informational literature, and creates correspondence advising of healthcare options.
        • Contacts patient by phone to follow up on scheduled appointments, no shows, or other planned interventions.

      • Works in conjunction with Population Health Coordinators and medical teams to coordinate care for patients across the continuum (inpatient, outpatient, community) to assure appropriate utilization of healthcare and community resources.
        • Contacts primary care physician to inform of patient discharge from other inpatient facilities.
        • Based on discharge information, may suggest need for follow-up appointment, additional services, or other medical care.

      • Works in conjunction with Population Health Coordinators and medical teams to refer patients to appropriate resources, within and outside the hospital system.

      • Completes tracking forms, and follows up on missed appointments and procedures.
        • Completes Patient Assessment Form for each patient.

      • Provides the healthcare team with patient self-management progress and health behavior changes.
        • Reports patient progress at team meetings.
        • Discusses alternative interventions and/or care plan revisions to best meet the needs of the patient.

      • Organizes data to provide meaningful understanding of the population and needs.
        • Tracks patient information as requested by Population Health Director.
        • Identifies trends and discusses with Population Health Team.

      • Assists with staff communication regarding contract terms or members.

      • Provides communication updates and routine reports to Population Health Director.

    • Essential Functions II

      • Participates in mandatory in-services and/or CE programs as mandated by policies and procedures/external agencies and as directed by management.

      • Follows and understands the mission, vision, core values, Employee Standards of Behavior and company policies/procedures.

      • Other duties as assigned.
