This position would provide case management services to Creighton University undergrad and graduate/professional students in Omaha as well as virtual or telephonic assistance to students at distance locations including the Health Sciences campus in Phoenix, Arizona. No travel is necessary. This position will work with clinical providers to assess student needs and help coordinate services both on campus and within the community and assist with case assignment. This position will manage and coordinate student support agreements and follow students who have temporarily left the university due to personal/mental health reasons and facilitate their return to campus with necessary supports. This position will be an advocate for students from diverse backgrounds and work with various departments across campus to maximize student success in the areas of mental health and wellness, academic success, referral services and campus transitions. They will work with community referral sources and community services, establishing formalized relationships with various community resources to meet a variety of student needs including but not limited to substance use, eating disorders, higher levels of care such as partial and inpatient hospitalization services. They will assist the Assistant Director for Student Care and Outreach (crisis manager) with hospital admissions and discharge planning. Additional duties will include documentation of student contacts, as well as assist with Outreach and creating resource materials to increase education on wellness and mental health resources on campus.Qualifications:
Minimum Bachelor's degree in Social Work.
1-3 years experience working with college age population or other similar case management population.
Knowledge, Skills, Abilities:
Proficient with electronic health record
Understanding of interworking of campus systems
Understanding of case management responsibilities in higher education
Ability to work and understand undergraduate and graduate/professional student population with variety of mental health needs
Ability to work within multidisciplinary treatment team
Understanding of FERPA/HIPAA regulations
