Description

Just like the children we serve, Children's is growing quickly and hiring a Critical Care Paramedic to provide high quality, individualized, family centered care to pediatric patients ranging from extreme prematurity through adolescence in the critical care transport environment. Care and transport of the critically ill patient is supported through the use of the Paramedic scope of practice in accordance with the Commission on Accreditation of Medical Transport Systems (CAMTS) standards and hospital policies and procedures. Varied schedules to include weekend and holiday patterns

  • Conducts individualized age-specific patient/family assessments according to defined paramedic standards, or protocols. (Individualized refers to the patient s developmental, cognitive, cultural, psychosocial, spiritual, and educational/family influences and physical/medical care needs.)
  • Initial Assessment:
  • Collects and documents data related to patient s individualized care.
  • Demonstrates age-specific competency in assessment of patients by the ability to obtain and interpret information in terms of the patient needs, knowledge of growth and development, and understanding of the range of treatment needed by these patients.
  • Assesses patient/family need for education and uses an individualized approach in delivery style.
  • Assess and interprets patient data upon arrival at the referring hospital.
  • Reassessment:
  • Performs reassessment based on unit standards, protocols and changes in patient condition. Communicates changes to appropriate persons (RN, NNP, MCP, family) as indicated.
  • Performs reassessments on transports after interventions and as needed. Communicates changes to transport RN, NNP or medical control physician (MCP).
  • Adheres to individualized patient/family plan of care to meet identified needs. (Plan of Care includes current and anticipated actions focused on stabilization, care, and transport needs).
  • Utilizes assessment data from the referral hospital or team as well as transport assessment data to individualize patient care and transport needs.
  • Utilizes individualized assessment data to develop the patient/family s plan of care.
  • Revises plan of care based on patient s condition and transport conditions (weather, distance, mode.)
  • Demonstrates ability to prioritize patient interventions based on assessments/reassessments.
  • While on transport, develops and initiates a plan of care incorporating current and potential problems requiring intervention, expected response and the medical plan of care in collaboration with the transport RN, transport NNP and/or the MCP. Communicates the plan to the receiving nurse prior to arrival at Children s Hospital or receiving facility.
  • Implements and evaluates actions in accordance with the individualized patient/family plan of care and patient condition/needs.
  • Incorporates critical thinking into the delivery of clinical skills and in analyzing patient response to orders, tests, labs, medications, and treatments.
  • Demonstrates clinical competence in administering interventions according to identified standards and age specific needs.
  • During the transport process, prioritizes and organizes care based on patient acuity, time limitations and consultation with transport RN/NNP or medical control physician. Initiates action to prevent current or potential risk to a patient in an emergency situation.
  • Documentation reflects accurate assessment/evaluation, modification, and patients response to therapy, changes in condition, and therapeutic interventions/procedures.
  • Entries into in the electronic medical record follow transport documentation policy.
  • Documentation is completed within established timeframes.
  • Demonstrates responsiveness to the organizational functions of the transport process.
  • Maintains Response Times within targeted benchmarks set by the program.
  • Complies with ambulance, helicopter, and fixed wing operational safety in accordance with current guidelines.
  • Checks, cleans, and stocks transport bags and equipment. Ensures equipment is in good working order.
  • Maintains all transport equipment in a state of mission readiness by checking all equipment at the beginning of each shift, noting any discrepancies and ensuring steps toward resolution.
  • Cleans equipment and restocks bags after a transport.
  • Demonstrates knowledge of transport equipment location, function and restocking procedures. Demonstrates the ability to troubleshoot all equipment and ensures that all malfunctions are promptly reported.
  • Completes daily, weekly, monthly and ongoing equipment checklists.
  • Completes quarterly competencies, monthly education, aircrew training, and other educational and professional development requirements when assigned.
  • Provides safe driving of the ambulance according to the Standards of the US Department of Transportation, the National Highway Traffic Safety Administration and EMS National Standards.
  • Utilizes lights and sirens as per Children s Policy adhering to standard safety practices.
  • Performs other duties as assigned
  • Accepts assignment to mentor EMT and paramedic students when scheduled.
  • Accepts assignments to share/teach knowledge and provide clinical support as a relief preceptor for new staff.
KNOWLEDGE, SKILLS AND ABILITIES:
  • Must be able to perform a variety of duties, often changing from one task to another of a different nature, without loss of efficiency or composure and be able to perform under stressful conditions when confronted with multiple requests, demonstrating prioritizing skills
  • Must be able to work in an environment which includes the use of ambulance, helicopter, and airplane patient transport vehicles that results in exposure to non-conventional motions, noise, vibration, fumes and extreme heat/cold.

EDUCATION AND EXPERIENCE:

  • Bachelor s degree from an accredited college or university in Emergency Medical Services or related health care field preferred.
  • Completion of a Paramedic program required.
  • Minimum 3 years of ALS experience as a Paramedic required including minimum 1 year of experience with pediatric patients required.
  • CERTIFICATIONS/LICENSURE REQUIREMENTS:
  • Current and Valid Nebraska and National Registry Paramedic license required.
  • Current and Valid Basic Life Support (BLS) required.
  • Current and valid Advanced Cardiac Life Support (ACLS) required, within 1 year of hire.
  • Current and valid Neonatal Resuscitation Program (NRP) required, within 6 months of hire.
  • Current and valid Pediatric Advanced Life Support (PALS) required, within 6 months of hire.
  • Current and valid Pre-Hospital Transport Life Support (PHTLS) or CAMTS approved trauma certification course preferred.
  • Current and valid Critical Care or FP-C Certification required, within 2 years of hire.

Special Job Requirements

  • Must have valid driver s license with an acceptable motor vehicle driving record required.
  • Completed and passed the Emergency Vehicle Operators Course required, within one year of hire
  • Must complete annual vehicle drivers acknowledgment.

FIT FOR TRANSPORT DUTY:

  • Must be able to wear seat belts installed in transport vehicle.
  • Must be able to lift and carry 50 pounds at least 100 feet.
  • Must be able to perform multiple physical activities at the same time (i.e. carrying equipment while pushing a cot or incubator)
  • Must wear flight suit appropriately at all times during shift.
  • Must wear over the ankle boots at all times during shift.
  • Must wear appropriate safety gear when indicated.
  • Must be able to tolerate environmental extremes including loud noise and vibrations.
  • Adherence to Transport Fit for Duty policy

Medical Conditions:

  • Transport team members may not donate blood within 24 hours of a work shift.
  • Transport team members must complete a personal risk assessment at the beginning of each shift and adhere to policy related to results.
  • Under no circumstances are transport personnel allowed to smoke or use smokeless tobacco products during the transport period.
  • Any medical condition that may interfere with transport assignments must be reported to the Transport EMS Manager.
  • A signed statement from a personal physician must be submitted to the Transport EMS Manager and Employee Health if a health condition develops that prevents the transport personnel from participating in the program.
  • The pregnant transport person is not required to remain on active transport status any longer than she or her private physician feels she may safely do so.
