We are searching for a Manager Home Health Pharmacy to provide leadership, direction and supervision for all Home Health Pharmacy related activities in order to accomplish organizational and departmental strategic goals and objectives.  Demonstrates mastery of knowledge and skills required in assessing, planning, implementing and evaluating complex care needs of pediatric patients with acute and chronic disorders.  Responsible for the day-to-day operations of the department and has 24-hour accountability for assigned area.

Defines operational standards, organizes department-level activities to achieve optimum service delivery/systems/care/processes and setting high performance objectives for the department.  Holds self and others accountable to meet stated objectives, outcomes, goals, timetables and commitments, adhering to standards even in the face of unforeseen circumstances.

  1. Implements procedures to maximize efficiency and effectiveness of operations while achieving standards. Responsible for operations of department, sets departmental standards, evaluates results and takes appropriate action.
  • Develops and implements goals, objectives, plans, policies and procedures to achieve desired departmental outcomes as established in conjunction with Pharmacy Director and the CHHC administrative team.
  • Actively participates in researching, developing and planning new service opportunities.
  • Oversees the planning, implementation and evaluation of approved projects.
  • Identifies, obtains and appropriately utilizes resources necessary to meet the identified goals including recommending budget amounts for operating expenses and capital purchases.
  • Establishes and monitors the expense, salary and capital budgets for the service line.
  • Collects, organizes and submits necessary workload statistics and financial data as requested by the Pharmacy Director in a timely manner.

  1. Responsible for establishing staff levels needed to meet the needs of the department and the patients served.
  • Selects/hires employees and trains and/or oversees the training on job related tasks.
  • Conducts employee performance management duties including completion and delivery of performance evaluations, setting goals, dissemination of information, coaching, addressing performance issues/routine correction actions, and recommending termination of employment.
  • Makes salary recommendations.
  • Submits notifications for personnel actions (e.g. status changes, terminations).

30%

  1. Acts as “Coordinator” for all services provided by the department. Assures that the needs of the customers are met.
  • Ensures that patient care needs are assessed and communicated with members of the healthcare team.
  • Orders and maintains inventory supplies and services for the provision of pharmacy services for Children’s Hospital & Medical Center
  • Adjusts quantities based on minimums or usage requirements utilizing appropriate supply chain management techniques
  • Allocates appropriate space and organizes storage of medications and supplies
  • Coordinates pharmaceutical returns and medication recalls, optimizing credit whenever possible
  • Reviews and authorizes payment of invoices, ensuring proper pricing and terms. 
  • Communicates with the Pharmacy Administrative team regarding pharmaceutical shortages, changes in products, deleted products and pricing updates.

  1. Responsible for the timely and accurate dispensing of pharmaceuticals, ensuring proper preparation, labeling, dispensing and billing.
  • Reviews medication orders and appropriately triages urgent, routine, and PRN medication orders
  • Understands and demonstrates appropriate order entry, preparation and documentation of the following pharmaceutical products. 
  • Appropriately reviews medication therapy and intervenes with healthcare team as warranted.
  • Evaluates medication order for appropriate medication selection, dosage, route, frequency, duration of therapy and drug interactions (drug-drug, allergy, drug-disease, drug-lab, drug-food) in relation to the patient’s clinical status.
  • Performs daily profile review to assess appropriateness of medication therapy as detailed in “a” in relation to patient’s ongoing and changing disease state and clinical status.
  • Appropriately and routinely utilizes formulary
  • Assures that patient related documentation, clinical and departmental reports are accurately completed, submitted and/or filed in a timely manner according to internal and external requirements.

 

Ensures availability 24 hours a day, 7 days a week.

 

Regular attendance at work is an essential function of the job.

KNOWLEDGE, SKILLS AND ABILITIES:
  • Knowledge of pediatric pharmacy care and home care
  • Excellent oral, written and communication skills with bilingual skills preferred
  • Ability to plan, direct and evaluate the work of pharmacy and internal nursing personnel
  • Ability to communicate effectively with individuals and groups in a wide variety of settings.
  • High level of initiative, self direction and accountability for action required.
  • Skill in promoting multidisciplinary collaboration.
  • Analytical ability necessary to conduct cost/benefit analysis, in-depth budget variation analysis as well as resolve complex operational and sensitive interpersonal issues.

           

EDUCATION AND EXPERIENCE:
  • Bachelor’s degree from an accredited college or university in Pharmacy or Pharm. D. required.
  • Minimum 5 years experience in IV admixture required.
  • Minimum 1 year supervisory or lead experience preferred.

 

CERTIFICATIONS/LICENSURE REQUIREMENTS:

  • A valid driver’s license and an acceptable motor vehicle record/driving record required
  • Current and valid Nebraska pharmacy license required.
