Children's is looking to hire a  Pharmacy Medication Reconciliation Specialist, Pharmacy that under the direct supervision of the Pharmacist, the Medication Reconciliation Specialist is responsible for working with patients and their families to collect and compile a current and complete patient medication list and aid in streamlining the medication reconciliation process for providers and nurses.

Essential Functions/Competencies

Compiles and completes, in the clinical information system, a current home medication list on patients as soon as possible for new admissions to the hospital & in the ED in the room on the computer, at the patient bedside.

•           Asks about all necessary medication sub-categories during medication interview to include prescription medications, over-the-counter medications, vitamin and nutritional supplements, as well as the primary source for this information.

  • Includes medication/supplement name, doseage form, strength, dose, day/time of most recent dose.
  • Enters compliance comments & last dose for all historical and prescribed medications on the medication list.
  • Verifies home med list information with the person providing the details by asking them to read the Medication List screen in Epic and confirm the accuracy and completeness of the information
  • Clarify as many uncertainties and compile as complete a medication history as possible utilizing patient and family/caregiver information, ambulatory pharmacy records, clinic records, long-term-care facility records and information from previous hospital encounters
  • Assist with identification of medications utilizing appropriate resources.
  • Refers appropriate questions to pharmacists in all cases of unresolved uncertainties related to a patient’s medication & allergy history..


Facilitates medication reconciliation workflow with providers and nurses in Epic to ensure all medications are reconciled in Epic within 24 hours of patient admission.


Addresses customer service issues in a timely manner, including answering phones, directing calls, greeting visitors, and responding to questions in the Pharmacy.


Perform other functions as described in the Children’s Hospital & Medical Center Pharmacy Technician job description as needed.


Regular attendance at work is an essential function of the job.

  • Exceptional customer service and communication skills across a variety of media.
  • Mathematical skills necessary for algebraic conversions, addition, subtraction, multiplication and division.
  • Previous experience in health care, with knowledge of medications, medication terminology.
  • Computer data entry skills.
  • Must be at least 18 years of age.
  • High school diploma or GED equivalent required
  • Minimum 2 years pharmacy experience required, experience in a retail pharmacy setting preferred.


  • Pharmacy Technician Certification from a recognized national or local certification board required.
  • Current and valid registration with the State of Nebraska required.


  • Successful completion of security check in compliance with requirements set by the State of Nebraska required.
