Job Description

Just like the children we serve, Children's is growing quickly and hiring a Medical Social Worker for the new Suvivorship clinic and Hemotology/Oncology clinic to provide professional social work services for patients and families. Enhance the psychosocial functioning of patients/families and continuity of care. Establish and maintain professional working relationships with hospital staff, CP staff, SPC staff, Home Care and other community agencies

Essential Functions/Competencies

20%

  1. Independently conducts psychosocial assessments:
    • Screens patients in assigned areas daily.
    • Performs psychosocial assessments, and treatment planning, including risk of self-harm, harm to others, and other mental health issues.
    • Collaborates with multidisciplinary team and Behavioral Health to understand the medical and mental health needs of the patients/families.
    • Appropriately documents assessment in chart.
    • Coordinates follow-up care, including recommendations for inpatient psychiatric care or out-patient counseling.
    • Provides appropriate patient education and documents appropriately.
    • Communicates psychosocial assessment and plan of care with patient care team.

  1. Provides referrals for patient and family needs.
    • Gathers information from patients/families.
    • Shares appropriate resource materials and information with patients/families.
    • Refers underinsured or uninsured patients to financial counselors. Provides assistance for medication prescriptions as deemed appropriate.
    • Researches resources specific to patient’s medical condition.
    • Assess for patients with frequent visits, readmissions, and non-compliance. Collaborate with healthcare team and make appropriate referrals.
    • Obtains necessary release of information authorization.
    • Provides follow-up as deemed appropriate.
    • Documents interventions appropriately in chart.

  1. Independently provides crisis/supportive counseling to patients/families
    • Assists families with problem-solving, adjustment to illness, bereavement counseling and management of long-term illness, utilizing their positive coping strategies.
    • Provides interventions which reflect knowledge of crisis/counseling technique and is enhanced by disease specific knowledge.
    • Documents interventions appropriately in chart. Provides recommendations and hand-offs to health care team members.
    • Facilitates support groups as needed.

  1. Independently provides assessment and coordination of child abuse/neglect cases.  
  • Assesses psychosocial dynamics.
  • Communicates with medical and nursing personnel regarding assessment and plan.
  • Facilitates referral and provides ongoing communication with CPS, law enforcement, and county attorney’s office.
  • Documents pertinent information and completes all reports in a timely fashion.
  • Prepares and presents cases to the Children’s Advocacy Team. Attends team meetings on an as needed basis.

  1. Provides on-call services for Social Work Department as a licensed, clinical Social Worker.
  • Assists with all Trauma Cases in the Emergency Department as a crucial team member.
  • Responds to calls in a timely manner.
  • Performs on-call duties in a professional manner.

  1. Covers for Social Work colleagues in their absence, and arranges for coverage of social work responsibilities when absent.
  • Communicates with co-workers regarding coverage needs.

  1. Provides consultation/interventions regarding legal issues such as determining guardianship.
  • Consults with Compliance Officer/Legal Counsel as necessary
  • Is a resource for others when the patient is a state ward

  1. Assists with mentoring of provisionally licensed Medical Social Workers and Social Work students.

 

Based on departmental/organizational need may be required to participate in Transplant program functions

 

Regular attendance at work is an essential function of the job.

KNOWLEDGE, SKILLS AND ABILITIES:
  • Requires knowledge of human behavior, social problems, counseling modalities, and community resources
  • Ability to communicate effectively both verbally and in written form, relaying case information to patients/families, physicians, and other healthcare professionals, attorneys and law enforcement as needed
 
EDUCATION AND EXPERIENCE:
  • Master’s Degree from an accredited college or university in Social Work required
  • Minimum three years’ experience of supervised experience as a Social Worker in a clinical setting required
  • Experience in medical social work preferred

 

CERTIFICATIONS/LICENSURE REQUIREMENTS:

  • Current and Valid Masters Social Worker and Mental Health Practitioner licensure in the state of Nebraska required
