Job Description

 

GENERAL SUMMARY:  Responsible for the day-to-day oversight and coordination of all activities of the HEROES program including adherence to departmental and organization standards and participation in the attainment of goals and objectives.  This position coordinates the multiple clinical sites of the program recommending/implementing process changes.  Provides leadership with accountability in promoting quality patient care and an engaging work environment.

Coordinates all facets and sites of the HEROES program in collaboration with medical director and leadership.

  • Performs in a leadership role to promote smooth and efficient operational flow of the HEROES clinic.
  • Facilitates communication between healthcare team members/departments in advocating for quality, coordinated patient care and ensuring that patient/family needs are met.  
  • Recommends staffing levels that match patient needs with staff competencies, and staff skill mix, through ongoing assessment of patient and program needs. 
  • Orients new staff.
  • Ensures that staff has the supplies, equipment, information, and resources needed to provide quality patient care in a safe work environment.
  • Coordinates research projects or PI projects in collaboration with medical director and HEROES team.
  • Participates in community outreach or marketing projects for the program
  • Uses logic and sound decision making in solving problems.
  • Follows through, communicates, and monitors individual/area issues.
  • Evaluates the clinical knowledge and skills of individual staff in a timely, positive, fair manner, and coaches to staff.
  • Manages time effectively, sets priorities, and delegates duties appropriately.
  • Contributes to the changes and improvement of patient care practices.
  • Participates in and supports staff in Shared Governance.
  • Utilizes disciplinary process effectively.
  • Monitors progress of orientees/preceptors.
  • Maintains systems to provide adequate supplies and equipment to area.
  • Works with manager and specialty/module providers and staff to prioritize and select Improvement projects.
  •  Actively participates and/or leads group working on project and documents and communicates progress and outcomes
  • Supports decisions of leadership team with staff.
  • Provides educational presentations as requested.
  • Assist families with appointments, testing, and orientation to the program at exercise sites.
  • Assists with parent information meetings for new referrals.
  • Coordinates educational programs for team members.
  • Serves a liaison to SPC administration and the HEROES team.
  • Coordinates care between the HEROES program sites.
  • In collaboration with the Bariatrics medical director and APRN, coordinates operations of bariatric portion of program.

  • Collaborates with manager to create and maintain an environment that advocates for staff and promotes individual/team engagement through clinical/administrative expertise and professional leadership.

  • Serves as a clinical/systems expert with nursing and interdisciplinary team member(s) in the assessment, development, implementation, and/or evaluation of the age-specific plan of care.  Plan of care includes current and anticipated actions focused on care education, and/or discharge needs.
  • Demonstrates clinical competence in an organized manner with speed, flexibility, and appropriate prioritization while maintaining quality patient care. Uses discretionary judgment in assessing conflicting priorities and needs.
  • Functions as an effective liaison between patients, families, staff, and other departments to foster a respectful clinical environment that promotes quality care.

  • Promotes an interactive learning environment with employees through coaching, mentoring, and providing opportunities to develop staff skills thereby enhancing job performance.  
  • Provides guidance for the employee’s development opportunities for their professional development.
  • Provides timely, ongoing and clear performance expectations, through constructive, consistent positive/negative feedback, even when the message is potentially difficult to hear. 
  1. Demonstrates and promotes professional leadership
  • Provides recommendations and collaborates with manager to improve systems and daily operations.
  • Makes recommendations and takes action to improve structures, systems, or outcomes based on analysis of current processes.
  • Acts promptly and decisively to address problems that arise on the unit, working collaboratively with others for guidance in decision making, initiating the chain of command as appropriate.
  • Mobilizes staff in emergency situations and maintains a calm demeanor while giving directions. Initiates discussion after the event to analyze how situation was handled and gives feedback for future emergencies.

  • Oversees the daily work of assigned staff within the specialties of the assigned module including assigning work, ensuring staff adherence to policies/procedures/practices, correcting and/or counseling staff on technical and work behaviors.
  1. Assists Ambulatory Manager with time card maintenance by entering information into Kronos for PTO, unscheduled absences, FMLA, etc.
  2. Provides documentation on performance and behavior for performance evaluations.  Participates in the performance evaluation meeting.

 

Regular attendance at work is an essential function of the job.

    • Accepts assignment to mentor student nurse observation experience in unit.
    • Accepts assignments to share/teach knowledge and provide clinical support as a preceptor/resource for new lead nurses.
    • Accepts assignment on unit to provide direct patient care as necessary, and actively seeks opportunities to help teammates/colleagues

KNOWLEDGE, SKILLS AND ABILITIES:

 

  • Must possess leadership skills in verbal/written communication and organizational management (time management, problem solving, conflict resolution, and knowledge of regulatory agency/HR standards). 
  • Must possess influential/motivational leadership skills that drive/develops staff performance.
  • Must be able to perform a variety of duties, often changing from one task to another of a different nature, without loss of efficiency or composure, and be able to perform under stressful conditions when confronted with multiple requests, demonstrating prioritizing skills.

 

EDUCATION AND EXPERIENCE:
  • Bachelor’s degree from an accredited college or university in nursing required.
  • Minimum 3 years nursing experience required, preferably in a clinic or pediatric environment

 

CERTIFICATIONS/LICENSURE REQUIREMENTS:

  • Current and valid Nebraska RN license/RN Compact license required
  • Basic Cardiac Life Support required in order to work
  • Nursing administration or unit specific certification preferred
