Establishes and coordinates the education for diabetic patients. The vast majority of patients seen are adults and geriatrics, occasionally adolescents. Provides liaison with the hospital dietary department and nursing. Participates in client-centered case management and coordinates multidisciplinary plan of care.
RD or RN:
RD - Graduate from an approved program in dietetics or nutrition.
RN - Bachelor of Science in Nursing required.
Preferred: Masters Degree. Certified Diabetes Educator.
Required: A minimum of three years of progressive experience in a hospital setting required.
Preferred: Experience with diabetic patients.
Position Requirement(s): License/Certification/Registration
Required: Current professional registration/licensure required.
Department Position Summary:
The employee must be able to demonstrate the knowledge and skills necessary to provide care appropriate to the age of the patients served on the various units. The individual must be able to coordinate and provide diabetes education to patients based on needs assessment.
The individual must demonstrate knowledge of the principles of growth and development over the life span, possess the ability to assess data reflective of the patients status, interpret the appropriate information needed to identify each patients requirements relative to his or her age-specific needs, and be able to provide the care needed as described in the departments policies and procedures.
The employee must be able to demonstrate the knowledge and skills needed to provide education to adult learners.
