Job Summary: The Patient Navigator is a healthcare service provider whose major responsibility is to reduce barriers to care for individual patients post discharge. He/She supports patients in finding the appropriate services that will aid the patient in maintaining their own healthcare outside of the hospital setting. The navigator maintains records of all assistance given and follows up with outside providers to monitor program efficiency and works with the care management director to improve the process. He/She collaborates with the physician in the emergency room to effectively assess the patients needs post discharge. Education: Required: High School Diploma or equivalent Preferred: Associates Degree in healthcare related field or current LPN licensure in the state of Tennessee Experience: Required: Excellent communication skills Computer literate. Knowledge of medical terminology and community resources Minimum of at least 3 years of hospital or related medical field experience. Preferred: Experience as Health Coach, Educator Position Requirement(s): License/Certification/Registration Required: Current license to practice Practical Nursing in State of Tennessee if applicable Preferred: Department Position Summary: The Patient Navigator functions on the multidisciplinary team in the emergency room setting as an advocate and coordinator of patients care in the outpatient setting. He/She recognizes, evaluates, and develops new services/processes based on the assessment and review of patients needs. He/She collaborates with the team to provide a supportive plan. The Navigator addresses the patients insurance coverage and assists with finding the patient a healthcare provider within the patients stated preferences. The Navigator may need to partner with the patients family or support team to coordinate post discharge care. He/She responds quickly and accurately to requests for information, resources, counseling and support. He/She maintains a positive relationship with referral sources. The Navigator maintains a patient database to measure the effectiveness of the program. He/She establishes a baseline for the program metrics and provides weekly progress reports.
