The goal of the Clinical Coordinator is to facilitate the transfer and admission of patients with a neurological diagnosis to the appropriate physician/service at Erlanger Health System, provide critical care nursing to patients with neurological diagnosis during procedures in the Emergency Department, Interventional Radiology Suite, and/or in the ICU (NNICU/MSICU). A Clinical Coordinator is responsible for providing care to patients with the diagnosis of stroke from the moment of arrival in the Emergency Department to placement in the ICU. The Clinical Coordinator is responsible for responding to all Code Strokes called from the Emergency Department and In-House Code Stroke pages. The RN Clinical Coordinator will be required to work as a key member of the multi-disciplinary team in the hospital providing physical, psychological and emotional support to stroke patients and their families through their acute phase of stroke disease. The RN Clinical Coordinator may act as a liaison between Stroke Services and other primary team members. The Clinical Coordinator is responsible for providing efficient and high quality service, respecting the needs of the patient; e.g. same day assessment of stroke patients including rapid tPA assessment and rapid access TIA referrals, establishing a history from the family and patient, management of acute complications and providing information and education to both the patient, staff, and family. The Clinical Coordinator facilitates transfer, performs the initial focused neurological assessment (if not already performed), provides critical care level nursing care, coordinates immediate medical management, activates the stroke and research teams, and supervises triage of all acute stroke admissions. The Clinical Coordinator also acts as a neuroscience clinical resource for the hospital.
Graduate from an accredited School of Nursing.
Three to five years of nursing experience , with two years of adult ICU or adult ED nursing experience. Knowledge of general nursing principles and practice skills; knowledge of organizational functions, policies and regulations; knowledge of current trends and new developments; knowledge of principles and practices of nursing, applicable to those areas requiring special education.
2 years experience in specialty; neurology experience

Current Licensure to practice nursing in the State of Tennessee.
BLS required within 30 days of hire.
ACLS required within 30 days of hire.

Department Position Summary:
The employee must be able to respond to all code strokes called through the Code Stroke Pager in effort to provide navigational support, appropriate 1:1 patient ratio, and ensure appropriate assessment is performed and that it is documented throughout the continuum per physicians orders and per best practice for Stroke care.
The employee must be able to demonstrate the knowledge and skills necessary to provide care appropriate to the age of the patients served who present with diagnosis of stroke or stroke like symptoms. The employee must be able to provide rapid tPA assessment and rapid access to tPA and additional stroke protocols. The employee must be able to manage time and caseload
The individual must demonstrate knowledge of the principles of growth and development over the life span and possess the ability to assess data reflective of the patients status and interpret the appropriate information needed to identify each patients requirements relative to his or her age-specific needs and to provide the care needed as described in the units policies and procedures.
The employee must possess the ability to assess patients for neurological deficits and the need for immediate intervention, follow the stroke patient through the continuum to ensure appropriate care is provided; select, develop, implement, and evaluate a plan of care. The employee must possess the ability to administer pharmacological drugs as ordered, perform procedures, and operate equipment necessary for patient care as assigned.
The employee must be able to perform same day assessment and follow up on patients who are admitted with stroke diagnosis. He/She must be able to accept appropriate referrals from members of the multidisciplinary team and other allied health professionals.
In collaboration with other staff, this employee must be able to follow the patient through diagnostic testing, clinical trial investigation which have set protocols, post interventional care, post IV tPA care, through the admission process, to include education regarding stroke and reason for admission, and discharge planning. The employee is responsible for arranging referrals to necessary services in consultation with the physician and other multidisciplinary team members.
The employee must be able to provide psychological support for patients and their families, communicate and assess needs for hospital stay, further education and potential needs at discharge.
The employee must actively promote the liaison and coordination of care for patients between stroke services, the hospital and the community.
The employee must act as a patient advocate, involving communication, negotiation and representation of stroke patients values and decisions in collaboration with other members of the Multidisciplinary Team.
Within the Interventional Suite, the Clinical Coordinators role is to help plan the safety and care of the patient post-procedure. The Clinical Coordinator must be able to assist the interventional radiology staff with the following:
recognizing the early signs of complications and acting upon these as appropriate
Maintaining patients airway
Observation of rate and depth of respirations
Maintenance of adequate circulation
Groin checks every 15 minutes for the 1st 2 hours post procedure, then q 30 x 6 hours, then q 1 hour x 16 hours, then routine (q2 in NNICU, q4 on NW7)
Pedal pulse assessment every 15 minutes for the 1st 2 hours post procedure
