Job Description

Mosaic is a faith-based nonprofit organization providing a life of possibilities for people with intellectual disabilities. Mosaic is currently looking for a Direct Support Professional to join our team in Terre Haute.

Responsibilities include:

Monitor the comfort and safety of individuals served, while ensuring that the medical, nutritional and personal care needs are being met.
Maintain medical, social and programmatic files by electronically recording each individual's progress and documenting other required information.
Provide training to individuals in accordance with the written program plan, covering all aspects of the individual's life as assigned and in compliance with company policy and procedure.
Support individuals served to identify and achieve their personal outcomes.
Assist individuals served with medication administration.
Attend all required meetings and trainings per agency needs.
Interact with individuals served in an emotionally supportive and therapeutic manner and practice effective techniques for reinforcing appropriate individual behavior.
Provide transportation and assist individuals served with accessing services and participating in community activities.
Assist individuals served with general housekeeping, meal preparation and laundry duties
Complete assigned cleaning tasks on a routine basis in order to maintain a neat and clean environment.
Assist individuals served in handling their money and finances.
Develop and support individuals in leisure or recreational activities.

Successful candidates will have:

High school diploma or equivalent, based on state requirements.
Minimum of three months of experience in a related field preferred.
Must be at least 18 years of age.
