Job Summary:

Provides direct and indirect client care under the direction of the Lead Nurse/RN and Program Director

Duties and Responsibilities:

  • Communicate in an effective & accurate manner verbally & in writing
  • Assess & Re-Assess physical & mental health status of all clients
  • Provide for the personal care and comfort of A& D clients
  • Provide a safe, clean environment
  • Orient clients to the unit
  • Perform and record vital signs for residential clients, notifies RN of any changes or out of range results
  • Demonstrate competency in administering Blood Glucose Tests, interpretation & proper documentation of results
  • Serve as a Client Advocate regarding medical & mental health issues
  • Display working knowledge of Infection Control Management per CDC Guidelines
  • Provide client care in a compassionate manner according to CADAS Treatment Protocols
  • Monitor client's compliance with the program requirements/rules
  • Continuously observe client's medical, behavior, mental status and activities, notifies RN, director and/or counselor of any changes or unusual occurrences
  • Demonstrate the ability to identify behaviors that require intervention
  • Escort clients when leaving the unit for activities and scheduled tests
  • Perform client safety checks at scheduled times, documents appropriately
  • Responsible for observation for high-risk clients
  • Communicate appropriately and clearly to the Director, RN's, and Counselors
  • Demonstrate the ability to set limits for clients and other de-escalation techniques
  • Documentation meets current standards and policies
  • Treat clients and families with respect and dignity, provides emotional support
  • Maintain a therapeutic relationship with Clients & families
  • Perform all aspects of client care in an environment that optimizes client safety and reduces the likelihood of medical/health care errors
  • Maintain appropriate levels of supplies for the unit
  • Answer the telephone in a professional manner, directs calls appropriately, and takes messages
  • Operate equipment safely and correctly
  • Maintain a good working relationship within the department and with the other departments of the facility
  • Demonstrate the ability to be organized and flexible, acts appropriately in stressful/emergency situations
  • Uses two identifiers for each client served to ensure identification
  • Protect client's confidentiality via utilization of HIPAA guidelines
  • Successfully completes Annual In-Service with a passing score
  • Support the Mission of the organization as well as that of the Nursing Department, "Always Do the Right Thing for Every Client"
  • Exhibit ethical behavior in dealings with clients/family, co-workers, as well as in the community at large
  • Participate in Unit Staff Meetings, as well as Nursing Staff Meetings.
  • Participate in organizational performance improvement projects, Core Measures, MOCS/ Measure of Client Satisfaction, etc.
  • Performs other duties as assigned

Regulatory Requirements:

  • Maintain current CNA/MA/EMT Licensure via the State of Tennessee, or currently enrolled in a nursing school with completion of the first semester
  • Current BCLS / First Aid Certification
  • Valid Driver’s License required

Language Skills:

  • Ability to effectively communicate in English, both verbally and in writing
  • Additional languages preferred


  • Basic computer knowledge
  • Excellent interpersonal skills
