The Community Support Provider provides client advocacy, ensures continuity of care, supports clients in time of crisis, provides/procures skills training, ensures the acquisition of necessary resources, and assists the clients in achieving community/social integration.

Supervisory Responsibilities: This job has no supervisory responsibilities.

Essential Duties and Responsibilities include the following. Other duties may be assigned.

Case Management:

  • Provides case management services to assigned Community Support/Substance Abuse (CSP/SA) clients.  
  • Participates in discharge planning for referred or active CSP/SA individuals hospitalized for mental health /substance abuse issues or in the Emergency Protective Custody Program.
  • Directly provides active rehabilitation and support interventions with focus on activities that promote living a clean and sober life.  This includes activities of daily living, education, budgeting, medication compliance and self-administration, relapse prevention, social skills, and independent living skills.
  • Participation in and reporting to treatment and rehabilitation providers on progress in the areas of  relapse prevention, social skill acquisition and application, education, and substance use/abuse.
  • Crisis/relapse prevention planning and support.
  • Visits and/or contacts clients in his/her caseload consistent with client and family needs.
  • Facilitates access by clients to necessary resources and services.
  • Ensures follow-through by clients on hospital discharge recommendations.
  • Arranges for and/or provides transportation for clients to and from service appointments.
  • Coordinates services to a particular client across agencies.
  • Assists clients in identifying and accessing all available community resources to support sober and independent living.
  • Maintains a case/production load as defined by Health and Human Services service definitions.
  • Required to be able to drive, sit for periods of time in a car and be able to get in and out of a car many times a day. 

Documentation: 

  • Documents all client-related activities in the services record.
  • Completes accurate and coherent documentation within timeframes defined by agency standards.
  • Assigned charts are kept current and accurate as defined by agency standards.
  • Works with Magellan to obtain authorizations for treatment and use the Magellan website.
  • Is competent in adhering to CFR42 confidentiality rules for substance abuse records.

Client Advocacy: 

  • Advocates for clients in securing needed resources and policy changes.

Crisis On-Call:

  • Provides 24-hour emergency coverage duties for the Community Support Program, in-office or after-hours, as assigned

Other: 

  • Maintains communication with service agencies to monitor follow-through by clients.
  • Communicates regularly with supervisor and other team members regarding assigned clients and areas of concern.
  • Participates in staffing, case conferences, and other meetings held by providers or agencies involving the client.
  • Participates in quality assurance and evaluation processes of the Program.
  • Must comply with Agency policies and procedures, including those regarding conduct, confidentiality, and record keeping.

Education and/or Experience  Bachelor’s degree or higher in psychology, sociology, or a related human service field is required, but two years course work in a human services field, and two years experience/training with demonstrated skills and competencies in treatment of individuals with chemical dependency is acceptable.

Certificates, Licenses, Registrations   Must be a Nebraska licensed driver with an insurable driving/safety record.

  • Possess knowledge of case management functions, signs and symptoms of substance use and abuse, and the physiological impact of chronic substance use and abuse.
  • Possess knowledge of specialized services/resources available to persons with chronic substance abuse/dependence, and for persons with a mental illness in addition to their chronic substance abuse/dependence.
  • Ability to work independently, to organize work and to determine priorities.
  • Ability to follow both oral and written communication.
  • Ability to communicate effectively, and to establish rapport with Agency personnel, clients and others on their behalf, and other service representatives.
  • Ability to communicate effectively in both oral and written format.
  • Willing to work evening and weekend hours.
  • Ability to pass an extensive background check.
  • If in substance abuse recovery, a minimum of two years continuous sobriety/ “clean time” is required.
  • Must be 21 years of age or older.
