Job Title/Position: Social Work/Social Services

Reports To: Director of Clinical Services

Job Description Summary:

Responsible for consultation and direction of Social Services in Hospice.  Provides orientation and education regarding Social Services to the Hospice Staff.  Develops, organizes and implements programs, policies and procedures, and counseling and recommends functions necessary to ensure overall operational viability.  Maintains performance improvement activities within the department and participates in IDG.

Essential Job Functions/Responsibilities:

  1. Assures that social service/counseling activities are integrated and coordinated within Hospice.
  2. Develops specific, measureable, realistic, and timely plans and objectives, which enhance social service delivery.
  3. Conduct a psychosocial assessment and reassessment on all terminally ill patients.  This includes pediatric, adolescent, geriatric and the general patient population.
  4. Answers questions and offers emotional support to patient and family, explains all aspects of hospice care during the election visit.
  5. Demonstrates knowledge of clinical social work.
  6. Treats patients and their families with respect and dignity.  Identifies and addresses psychosocial needs of patients and families.
  7. Assist with symptom management through utilizing counseling, education, or short-term psychosocial techniques with patient/family related to anxiety with impeding death/loss.
  8. Explore ethical dilemmas and aide in problem solving, advocating, and facilitating the proper resources to develop solutions.
  9. Navigate financial concerns and assist in accessing available resources.
  10. Interacts professionally with patient/family and involves patient/family in the formation of the plan of care.
  11. Communicates appropriately and clearly to physicians, staff, Clinical Nurse Manage and administrative team.
  12. Coordinates and directs Social Services to ensure patients’ needs are met and hospice policy is followed.
  13.  Performs all aspects of patient care in an environment that optimizes patient safety and reduces the likelihood of medical/health care errors.  Supports and maintains a culture of safety and quality.
  14.  Manages and operates equipment safely and correctly.
  15.  Demonstrates an ability to be flexible, organized and function under stressful situations.
  16.  Maintains a good working relationship both within the department and with other departments.
  17.  Consults other departments as appropriate to collaborate in patient care and performance improvement activities.
  18.  Maintains performance improvement activities for department.
  19.  Documentation meets standards and policies.
  20.  Provides education to staff on performance improvement.  Ability to plan and organize orientation and in-service training for staff members.  Participates in guidance and educational programs.
  21.   Performs consultative activities including recommendations for changes in service delivery.

Position Qualifications:

  1.  Current State Clinical Social Work Provisional/Licensure or suitable degree in related field.
  2. Three (3) or more years of previous experience in all aspects of social work and discharge planning.
