We are looking for a dependable, compassionate people to provide direct oversight of our Direct Support Professionals (DSPs) in residential settings. Hands of Heartland has been supporting people with developmental disabilities since 1997. Job Description:

Team Member Coordinators manage assigned facilities to ensure adequate staff coverage and quality services to the people we support. Team Member Coordinators attend interdisciplinary team meetings and coordinate day-to-day activities within the residence. Team Member Coordinators must be able to work evenings and have flexible schedules to accommodate occasional rounds on various shifts and provide on-call coverage.

Duties and Responsibilities:

  • Provides oversight and direction to assigned team
  • Ensures services are provided to address the wants and needs of people with developmental disabilities as written in each person’s Individual Support Plan (ISP)
  • Ensures all documentation is completed in a timely manner including but not limited to incident/injury reports, Medication Administration Records, and health tracking
  • Safeguards individual’s funds, assists individuals with purchases, and accurately records expenses
  • Ensures all monitoring is completed and documented in Therap
  • Ensures assigned facilities and vehicles are clean and compliant with HOH and state requirements
  • Participates in the orientation and training of new team members
  • Collects the month end documentation and ensures all forms are available for the next month
  • Ensures communication book documentation is complete and appropriate
  • Ensures assigned team members are recording their time accurately and approves timesheets
  • Takes measures to reduce overtime
  • Interviews and fills open positions in a timely manner
  • With the assistance of HOH management, creates and implements team member appreciation initiatives


  • At least two years of experience working with people with developmental disabilities (education may be substituted for experience)
  • Ability to be on-call and travel locally based on operational needs
  • Excellent skills in communication, observation, problem solving, and time management
  • Ability to handle a variety of situations and personalities
  • Valid driver's license
