POSITION SUMMARY:
Responsible for connecting students with supports and services focusing on Education success. Assisting families with accessing community resources focused on Education, Employment, Housing, Finances, Transportation and Mental/Physical Health services. Responsible for creating Individualized Education Success Plans for at-risk youth and being a connector to resources for students and families.
ESSENTIAL DUTIES AND RESPONSIBILITIES:
Coordinates efforts between local school district and community service providers/partners, Community Response/Bring Up Nebraska collaboratives in the Omaha metro area, and the student and family to create Education Success Plans that contain the most appropriate supports and services available in a timely manner.
Develop, conduct, and implement Central Access consultations, ensure all necessary documentation is completed and entered into the database, and refer for any additional services deemed necessary.
Facilitate PEO orientation sessions.
Works with students and providers to ensure student and family friendly intake procedures when accessing services and supports.
Create and maintain relationships with Project Everlast Omaha program partners and community partners to provide the best coordination of services for students.
Coordinates with Douglas County Community Response (DCCR) and Lift Up Sarpy County to ensure a streamlined central navigation system and process for students and/or expectant and parenting students and their families.
Track student overall outcomes as it relates to grant reporting.
Work to identify supports and resources to assist students in overcoming barriers to academic success.
Assists in the planning and facilitation of PEO Youth Activities.
Attend Project Everlast, Partner Programs, and Community meetings as needed.
Participate in statewide Bring Up Nebraska and Connected Youth Initiative meetings and workgroups as needed or requested.
Develop and maintain database reporting and functionality in partnership with PEO Staff and partners.
Assist with the development, outreach and coordination of housing, transportation, and maternity programs.
Connect and partner with school personnel to develop a network of support.
Other duties may be assigned.

EDUCATION AND EXPERIENCE:
Required:
Valid Driver’s License and proof of insurance.
Any equivalent combination of the following experience, training, and/or education will be considered.
Associates degree in related field or
3 years’ experience in working with in juvenile justice, child welfare system, or an education/school setting
Demonstrate extensive knowledge relating to supports and services available for youth and families.
Lived experience in these systems of care.
Knowledge:
Working knowledge of Microsoft office suite including Word, Excel, PowerPoint and Outlook
Office equipment and computer software

Preferred:
Bachelor’s Degree in social work, human service, public administration, education, criminal justice, or similar degrees
LANGUAGE SKILLS:
Ability to read, analyze and interpret documents such as grant solicitations or governmental regulations.
Ability to write grant proposals and progress reports as well as routine reports and correspondence.
Ability to respond to common inquiries about the organization from the public and/or business community.
Ability to effectively present information to public groups.
MATHEMATICAL SKILLS:
Ability to add, subtract, multiply, and divide in all units of measure, using whole numbers, common fractions, and decimals.
Ability to compute, rate, ratio, and percent and to draw and interpret bar graphs.
Ability to read and interpret demographic and statistical research data.
REASONING ABILITY:
Ability to solve practical problems and deal with a variety of variables in unique situations.
Ability to interpret a variety of instructions furnished in written, oral, diagram, or schedule form.
Ability to define problems, collect data, establish facts, and draw valid conclusions.
Ability to interpret statistical research findings (such as Census data) and to interpret mathematical data accurately in written communications.

PHYSICAL DEMANDS:

WORKING CONDITIONS:
Flexible schedule including possible evenings and weekends.
