DEPARTMENT FUNCTION/JOB OVERVIEW

Responsible for data collection for the CMS Inpatient Rehabilitation Facility (IRF) and Madonna Rehabilitation Specialty Hospital (MRSH-L) Quality Reporting Program. Promotes an excellent patient experience through early customer service and service recovery efforts on the hospital units. Major responsibilities involve quality data collection and entering data, customer service including assisting to resolve patient concerns in a timely manner, discharge follow-up, communicating concerns to appropriate personnel, and presenting ongoing educational activities. Work will be performed in an ethical and legal manner following organizational policies, processes, protocols and procedures. Responsible for quality service delivery and internal/external customer relations for Madonna as a whole, including upholding the mission and values for the department and facility.

 
ESSENTIAL FUNCTIONS
 
(INFORMATION)

  1. Understands and utilizes Madonna programs, policies and procedures.
  2. Compiles and organizes information from the medical record and clinical sources in order to complete the LTACH CARES Data Set in LTRAX.
  3. Coordinate, compile, compare and record patient care information.
  4. Compile, compare, report and record patient data, patient care, and treatments of cares rendered.
  5. Contributes to the development and continuous improvement of mechanisms for plans to be recorded and retrieved.
  6. Facilitates the integration of quality patient care systems including components related to risk indicators, rehabilitation outcome indicators, high acuity indicators, emergency care, and return to acute care.
  7. Contributes to the integration of nursing quality management which is the link between operational data and research data with the goal of establishing evidence-based practice to improve patient care.
  8. Assists and supports staff in developing and maintaining competency in the planning and change process.
  9. Collaborates with nursing/patient care and risk management systems to ensure program compliance with regulations specific to nursing care.
  10. Schedules transportation.
  11. Coordinates documents for physician office visits.
  12. Inventories office and medical record form supplies, orders the supplies, and organizes supplies.
  13. Maintains each patient's medical record: files patient data in chart holder in prescribed order, writes patient name on chart forms and places in chart holder as needed.
  14. Accurately processes all aspects of the physician order transcription.

  15. Fosters a professional environment.
  16. Assists nursing and the interdisciplinary team in identifying patient and family needs, and provides patient and family with emotional support.
  17. Assists in identifying and resolving patient/family concerns in a timely manner to enhance the overall patient experience.
  18. Collaborates with the Education Instructor Customer Liaison to analyze patient satisfaction responses to enhance service recovery efforts and a positive patient experience.
  19. Communicates with interdisciplinary team to arrive at decisions for patientâ??s plan of care.
  20. Serves patient by assisting with transfers, wheeling in wheelchairs, ambulating and repositioning, ADLâ??s and feeding of patients.
  21. Speaks-signals to patient and family by answering call lights, responds in a timely respectful manner to maintain a positive customer relation.
  22. Follows instruction from supervisor to perform other functions as assigned in order to achieve the goals within the department and clinical services systems.
  23. Maintains privacy, confidentiality, and security of patient, client, staff, and organizational data.
  24. Collaborates with nurses providing direct patient care and members of other disciplines at all levels in the development, implementation, and evaluation of programs and services.
  25. Provides the opportunity for ongoing communication between self and staff.
  26. Serves as a contributing member of assigned committees to address needs/goals identified to ensure continuous clinical improvement.
  27. Communicates effectively with co-workers, education coordinator, program directors, supervisors, and other departments as required to disseminate necessary information accurately.

  29. Coordinates activities, schedules, and services for the patient.
    (THINGS)
  30. Sets up admitting patient rooms with proper safety equipment and completes a final environmental check of the room before patient enters.
  31. Sets up, maintains and ensures a clean and safe environment for the department.  Handles and operates all necessary equipment and perform required duties according to established safety standards to maintain compliance with regulations and prevent injury.
  32. Assists with ensuring cleanliness of equipment and environment.
  33. Must be able to operate a copier, telephone, fax machine and have basic computer skills, including email, word processing and spreadsheet functions.
    Medical devices used in work include, but not limited to:

                Oral/Tympanic thermometer
                Bed scale
                   Workstation on Wheels (WOW)
               transfer/repositioning devices
                trauma tub
                   Beds, wheelchairs, stretchers
                   Digital wheelchair/Standing scale

    PHYSICAL DEMANDS AND ENVIRONMENTAL CONDITIONS
     
    Medium work with frequent lifting.  Manual patient transfer or reposition which does not exceed the equivalent of 35 pounds of force to lift, push or pull. Requires sufficient strength to produce 50 pounds of force to push a wheelchair with a 300-pound individual on level surfaces. Constant walking with maximum duration of up 30 minutes sustained walking on tile, carpet and other surfaces. Must tolerate frequent standing with sustained periods of up to 1 hour on tile, carpet, and shower surfaces. Must tolerate frequent sitting with sustained periods of up to 1 hour.  Frequent reaching with both arms at low, waist, and high levels. Frequent grasping of both hands for transfers and repositioning requiring normal grip strength based on national averages. Frequent stooping, bending, and twisting are required for patient care. Constant near visual acuity to evaluate and monitor patients.  Ordinary hearing and speaking skills.  May be exposed to biohazardous materials. 
     
     
    QUALIFICATIONS (Education/training and/or Experience)
     
    Must possess current Nebraska LPN licensure. One year relevant work experience preferred, ideally in rehab nursing. Demonstrate the ability to interact in a positive and effective manner with staff, patients, and families either in person or by phone. Models Madonna's Core Values through behavior and performance of duties. Excellent communication skills and ability to interact in a positive manner either in person or by telephone.
