Home Health Care Physical Therapist

  • Our clinical staff members are paid per visit and are able to schedule client visits in a manner that fits into their own schedules.
  • Must be licensed in Nebraska.

    Essential Responsibilities:

    1. Utilizes knowledge and skills of Physical Therapy to evaluate, establish and implement individualized programs of preventive, restorative and maintenance physical therapy. 
    2. Provides physical therapy services to clients according to a written physician’s plan of care.  This may include, but is not limited to:
      • Assessing and evaluating therapeutic/rehabilitative/functional status and participation in the development of the total plan of care.
      • Evaluating home environment.
      • Assessing for muscle strength, mobility, gait, and ROM.
      • Directing physical therapy treatment.
      • Instructing clients and families/caregivers in the use and care of therapeutic appliances.
      • Determining priority needs for physical therapy.
    3. Initiates physical therapy program and instructs the client, other personnel and/or client caregivers in certain, appropriate phases of physical therapy as well as providing instruction pertaining to the goals of the physical therapy program for the client.
    4. May train client in the use of prosthetic devices.
    5. Supervises home health aids when such personnel participate in client’s rehabilitation regime.
    6. Identifies client and family/caregiver needs for other home health services and makes appropriate referrals.
    7. Prepares and submits a clinical progress summary based on the attainment of goals.
    8. Provides in-service education programs for agency personnel as needed.
    9. Secures and maintains medical equipment and/ or assistive living devices relevant to client’s needs/conditions.
    10. Directly supervises the therapy provided by Physical Therapy Assistants in accordance with State and Federal regulations if needed.
    11. Adheres to infection control policies and guidelines.
    12. Maintains professionalism while representing PCHHC as an employee.
    13. Adheres to company documentation and care procedures and standards of personal and professional conduct.
    14. Provides client assessments and evaluations and responds as indicated to changes in client condition.

Hours 32-40 hours per week; full time status working Monday through Friday. Salary based on number of visits made per week.
