Job Summary: (PPA)Provides parenting and adoption education and support to women, men and their families dealing with an unplanned pregnancy. Conducts support groups for women and men who choose to parent or choose to make an adoption plan. Conducts home studies, provides education and conducts support groups for prospective adoptive parents or adoptive parents. Conducts post-placement visits and post placement services which includes education and support to adoptive families. Presents to schools, other agencies, physicians/nurses, service organizations and others on services provided by NCHS. Presents to prospective adoptive parents and adoptive families on issues surrounding adoption at educational trainings offered through NCHS.
(RT) To collaborate with eligible adoptive or guardianship families to identify desired outcomes, needs to reach those outcomes and strategies to meet the needs or remove the barriers. The intent is to help preserve families by addressing needs most often expressed by families. Family networking services include respite care, mentoring, support groups, educational groups, and online services.
Essential Duties and Responsibilities (PPA):
- Provides education and support to women, men and their families on parenting and adoption by:
- Responding in a caring, understanding, prompt manner to all correspondence, telephone calls, and other inquiries.
- Assisting women and men with providing information on obtaining services and resources such as medical care, housing, educational, vocational, employment, child care, state assistance, financial, etc;
- Assisting women and men with providing education, within NCHS guidelines, on the options parenting and adoption by using decision making tools, budgets, the NCHS parenting curriculum, videos pertaining to parenting and adoption and using other articles and books.
- For those women and men considering adoption: obtaining social and medical information on mom and dad; sharing prospective adoptive parent profiles based on the mother and/or the father’s requests; facilitates any pre-birth meetings between the prospective birth parents and prospective adoptive parents; assists in completing the required legal paperwork and required NCHS forms to make an adoption plan, facilitates the adoptive placement of the child and provides appropriate social background and medical information to the adoptive parents.
- Collaborates with other caseworkers in the placement of children with special needs.
- Participates in special needs adoptive parent preparation meetings.
- Applies for SSI and subsidy for special needs adoption as needed.
- Conducting groups as necessary in regards to parenting and/or adoption, including post relinquishment support groups.
- Informs clients of legal aspects and documentation requirements.
- Writes post-placement reports.
- Maintains agency records related to dictation, NCHS required forms and legal documents pertaining to adoption planning and placement, including affidavits, notice to birth fathers, SSI and subsidy applications.
- Conducts home studies on prospective adoptive parents as assigned in accordance with Federal and with the State of Nebraska laws and NCHS guidelines by:
- Facilitates the education of prospective adoptive parents regarding adoption issues;
- Answers questions and assesses prospective adoptive parents readiness for parenting;
- Providing a written home study.
- Conducts support groups for prospective adoptive parents and/or parenting groups for adoptive parents and other educational groups.
- Providing education and support to adoptive parents post placement by:
- Conducting post placement visits and writing post placement reports for finalization.
- Facilitates the ongoing contact between adoptive parents and birth families, as needed.
- Networking with other agencies by:
- Provides schools, service organizations, physicians, nurses, hospital social workers, social service agencies, with brochures and other materials on the services provided by NCHS.
- Creates and maintains working relationships with other social service agencies by attending community social service meetings.
- Provides Presentations to groups, schools, agencies, physicians, nurses, hospital social workers and others to provide education on NCHS services and on how to present adoption as an option.
- Creates and Develops opportunities for program development and/or expansion by:
- Develops relationships with community agencies, to include the Department of Health and Human Services local offices, to identify needs and/or gaps in services in the community(ies) and provide the PPA Program Director; Asst. PPA Program Director; Director of Foster Care; Director of the Children and Family Center; and/or the Chief Program Officer with this information as a means of program development or enhancement.
- Develops work plans/action plans to implement program development as directed by the above Program Directors, Chief Program Officer and/or Chief Executive Officer
- Acts as a liason to community agencies in facilitating program development as requested by the above Program Directors, Chief Program Officer and/or Chief Executive Officer
- Attending educational trainings or workshops to provide ongoing professional development and to maintain individual license(s), if applicable.
- Adheres to confidentiality guidelines.
- Demonstrates respect, appreciation, and sensitivity for diversity.
- Participates within Continuous Quality Improvement program.
Essential Duties and Responsibilities (RT):
- Provide case management services to eligible children and families in the Right Turn Program.
- Utilize assessment tools to focus services toward more effective approaches for adoptive families
- Respond to crisis, correspondence, telephone calls and other inquiries from adoptive/guardianship family members in a prompt, caring and understanding manner. Availability to respond to crisis 24/7.
- Meet with the family a minimum of once per month and maintain phone contact on an ongoing basis.
- Support adoptive/guardianship families in addressing behavioral or other problems within the home through phone contact, face-to-face visits and referral/resource information.
- Advocate for the family in seeing that all physical and emotional needs are being met.
- Work with supervisor to develop and implement a service plan, organize and facilitate case reviews with all interested parties, providing input on provision and coordination of services.
- Reinforce and support adoptive/guardianship parents’ efforts to carry out the service plan and intervention strategies with their children.
- Arrange and/or monitor services provided to the family.
- Assess when family is ready for discharge from case management services.
- Collaborate with other community professionals working with adoptive families.
- Organize and facilitate networking opportunities for the family.
- Under supervision of the Right Turn supervisor, work with a NFAPA volunteer in the facilitation of adoptive family support groups for adults and youth.
- Educate families or designated support persons to provide respite care in a reciprocal, cooperative arrangement.
- Under supervision of the Right Turn supervisor, work with a NFAPA volunteer to educate, support and facilitate family to family mentoring.
- Under supervision of the Right Turn supervisor, develop and facilitate educational classes for adoptive parents or guardians.
Program Documentation and Outcome:
- Maintain timely, legible records of services provided per established program and Agency standards.
- Participate in Right Turn program evaluation and Continuous Quality Improvement activities as assigned.
Professionalism and Confidentiality:
- Keep all client names and information and other agency business confidential.
- Guard client confidentiality in waiting areas, during phone contact, and in handling of records.
- Insure all office and record security procedures are followed.
- Maintain a positive, respectful demeanor on all contacts with staff and the public.
- Seek needed guidance in handling difficult client situations and establishing priorities for the job.
- Work to build positive, productive relationships with other staff.
- Other duties as assigned.
Marginal Duties and Responsibilities:
- Performs other duties as assigned by the Pregnancy, Parenting and Adoption Program Director or Asst. Pregnancy, Parenting and Adoption Program Director or Right Turn Supervisor.
Qualifications and Requirements:
- Minimum education and experience (PPA):
- Master Degree preferred in Social Work, Counseling or other related Human Service Field; PLMHP or LMHP preferred
- Bachelor Degree in Social Work, Counseling, or other related Human Service Field
- One (1) to three (3) years experience in the pregnancy, parenting or adoption field with direct client contact
- An equivalent combination of education and experience will be considered.
- Minimum education and experience (RT)
- Bachelor’s degree in human services is preferred.
- Minimum of two years child welfare experience, with at least one year experience in issues pertaining to adoption.
- Valid Nebraska driver’s license, proof of liability insurance and ability to drive for Agency business.
- Knowledge, skills, and abilities:
- Knowledge of unplanned pregnancy, parenting, adoption and related issues
- Ability to manage a high volume caseload with multiple demands
- Ability to follow regulations and procedures
- Ability to read, discern and interpret information given by pregnancy, parenting and adoptive parent clients
- Ability to observe situations with sensitivity and objectivity
- Strong interpersonal skills; excellent verbal and written communication skills
- Basic computer skills, with willingness to learn and utilize NCHS software and hardware systems
- Ability to work independently, self-manage and work toward goal achievement
- Possess driver’s license and personal vehicle insurance
- Physical demands:
- Regularly required to speak and hear, walk, climb, sit, use hands and fingers to operate handle or feel objects and equipment
- Manual lifting requirements are medium.
- Able to operate a licensed vehicle for travel and possible transportation of clients .
- Schedule demands (travel, adjustments to accommodate clients, weekends/evenings, etc.):
- Must be able to adjust normal work schedule to accommodate the needs of clients, including possible evening and weekends.
- Site factors (work location, environment, travel, etc.):
- Location of this position is in an office setting.
- Local travel will be required with State-wide travel on occasion for meetings.
