Pathways to Permanency Specialist I

Job Summary:

The Pathways to Permanency Specialist will utilize the Pathways to Permanency model to empower families to provide a safe and nurturing environment for their children and increase overall positive functioning.    In addition to providing comprehensive education and support to families, the Pathways to Permanency Specialist will supervise parent/child and sibling interaction.  The Pathways to Permanency Specialist will maintain a collaborative role with the family's professional team to ensure comprehensive and consistent service.        

Essential Duties and Responsibilities:

  1. Provide home-based support and education to families using the Pathways to Permanency comprehensive support and education model including:
    1. Utilizing a trauma informed and strength based approach in all interactions with families and children;
    2. Provide oversight and education during parenting time between children and parents and during sibling visit times;
    3. Provide transportation of parents and children as needed to assist with achieving family goals;
    4. Driven by the family and in collaboration with case professionals, create a comprehensive  service plan to identify and outline family goals and timeframes for achievement;
    5. Collaborate with case professionals to ensure comprehensive service;
    6. In collaboration with the family, complete required assessments and documentation in    accordance with agency policies and procedures;
    7. Respond to correspondence, telephone calls, and other communication from families and case professionals in a prompt and professional manner;
    8. Attend all court hearings and meetings with the professional team to support the family and   provide objective feedback on family progress towards goals;
    9. Assist families in engaging and utilizing their informal support system;
    10. Demonstrate a flexible work schedule to meet the needs of families. 
    11. Complete ongoing training to ensure professional development


  1. Perform agency citizenship and public relations for the good of NCHS by:
    1. Facilitate a team atmosphere by working in partnership with other agency staff and promote a positive working environment;
    2. Maintain a positive relationship with state or private social service agencies, representing NCHS positively in the community;
    3. Assisting in continued compliance efforts with state licensing and COA standards.

  1. Participate within Continuous Quality Improvement program.


  1. Maintain a safe driving record with no more than three points on his/her driver’s license, with appropriate automobile insurance based on agency guidelines, and will successfully complete a defensive driving course, sanctioned by the National Safety Council, prior to beginning service delivery.

Qualifications and Requirements:

  1. Minimum education and experience: 
    1. Bachelor’s degree in social work, counseling or other related human services field required;
    2. Two (2) years of experience working with children and families; 
  2. Knowledge, skills, and abilities:
    1. Knowledge of the Nebraska foster care system and related issues;
    2. Knowledge of the impact of trauma on family systems;
    3. Knowledge of basic child development;
    4. Belief that people have the capacity to grow and change;
    5. Ability to develop respectful relationships with persons served to help them gain skills and confidence;
    6. Ability to maintain a helping role, intervene appropriately to meet service goals, and set appropriate limits;
    7. Ability to work autonomously and make appropriate decisions in crisis situations;
    8. Ability to interact with families with sensitivity and responsiveness to cultural and socioeconomic characteristics;
    9. Ability to read, analyze and interpret forms, documents, evaluations and procedure manuals;
    10. Ability to respond to inquiries and possible complaints from families and other providers;
    11. Ability to define problems, collect data, establish facts and draw valid conclusions.
    12. Commitment to trauma informed and strength based services;
    13. Ability to manage a high volume caseload with multiple demands;
    14. Ability to follow regulations and procedures;
    15. Ability to read, discern and interpret information given by families and other case       professionals;
    16. Ability to observe situations with sensitivity and objectivity;
    17. Strong interpersonal skills;
    18. Excellent verbal and written communication skills;
    19. Basic computer skills, with willingness to learn and utilize NCHS software and hardware systems;
    20. Ability to work independently, self-manage and work toward goal achievement;
    21. Possess a valid driver's license with no more than three points assessed;
    22. Possess appropriate automobile insurance based on agency guidelines;
  3. Physical demands:
    1. Regularly required to speak and hear, walk, climb, sit, use hands and fingers to operate, handle or feel objects and equipment.
    2. Manual lifting requirements are medium. 
  4. Schedule demands (travel, adjustments to accommodate clients, weekends/evenings, etc.): 
    1. Must be able to adjust normal work schedule to accommodate the needs of clients, including evening and weekend work.
  5. Site factors (work location, environment, travel, etc.): 
    1. Work is done in a combination of the office, family home, and in the community;
    2. Local travel is required.
