Supported Employment Assistant/Day Services Assistant

Implement individualized performance plans developed for clients.  Perform training on employability practices and model those practices.  Train Consumers on specific job skills and employer’s personnel policies, practices, and procedures.

Shared position between Day Services and Supported Employment.

Minimum Qualifications:

  • Must be at least 19 years of age
  • High school or equivalent diploma.
  • Ability to perform essential functions of this position with no more than reasonable accommodation as determined by Board of Directors.
  • No convictions for abuse, molestation, theft, burglary, robbery, assault, rape and other crimes against another person or property judged to be job-related.
  • No founded reports of abuse of a dependent person.
  • A satisfactory driving record (no more than three moving violations in the most recent thirty-six month period) and a valid driver’s license appropriate for vehicle type being required to operate and maintain.
  • Reliable personal transportation for job performance as may be occasionally required and having that personal transportation insured for liability, medical, and uninsured and underinsured motorists; proof of insurance must be submitted and maintained.
  • Not actively using illegal substances.
  • Associates Degree, or 6 months experience performing similar functions in a similar organization.

 Essential Functions:             

  • Refer qualified consumers to employer or employment service for placement.
  • Implement plans developed by and with the inter-disciplinary teams and other appropriate professionals.
  • Perform training and appropriate modeling of employability practices.
  • Provide direct service hours to consumers.       
  • Learn job skills used as training opportunities and teach those skills to clients.           
  • Learn and teach appropriate policies and procedures of employers to clients placed with those employers.
  • Maintain accurate and complete client file information in timely fashion.                 
  • Consult with supervisor concerning program plans for consumers.     
  • Assist with job-seeking skills such as application, interview, etc.          
  • Submits and reviews reports with supervisor.                           
  • Attend training events as required.                                                                                
  • Attend IPP Individual Program Plan and/or ICP Individual Consumer Program and/or Individual Education Plan IEP meetings as required
  • Maintain a safe and hazard-free environment. 
  • Hours are subject to change


Physical Requirements:

Seeing: Must be able to consistently read reports and use computer

Hearing: Must be able to consistently hear well enough to communicate with co-workers

Standing/Walking: Must be able to stand and walk frequently

Climbing/Stoop/Kneeling: Must be able to climb, stoop, and kneel occasionally

Lifting/Pulling/Pushing: Must be able to lift, pull, and push occasionally

Fingering/Grasping/Feeling: Must be able to finger, grasp, and feel constantly

Must be able to write, type, and use phone system.

Occasionally lift floor to waist 10 lbs to 50 lbs 1x each

Occasionally lift waist to shoulder 10lbs to 50 lbs 1x each

Push/Pull 30 lbs
