Care Coordinator

Vandenack Weaver Truhlsen is seeking a Care Coordinator to be a part of our Life Care Planning Team. We are a law firm that help individuals and families prepare for and respond to a variety of life transitions.  The Care Coordinator will be an advocate for our clients through the aging process, synchronize health care needs that arise, and help coordinate a safe plan to promote quality of life. We are looking to hire an empathetic individual that is compassionate and dedicated. 

Responsibilities:

• Assess client needs for living arrangement and gather information on medical status, resources, and support system
• Review insurance (all types), providing Medicare consultation and education
• Present care options, monitor client’s care, attend client’s doctor visits working with their healthcare team to ensure client and family understands the care
• Assist with health care reimbursement claims process
• Assist with understanding medical bills and making calls to ensure the proper insurance has been billed
• Assist with understanding pharmacy services, options, and billing
• Manage client transitions to include hospital visits, assist with admissions to hospital, other facilities, hospice
• Assist with Medicare waiver process
• Attend tours of facilities
• Attend care plan meetings and family meetings with care providers
• Work closely with our attorneys to assist the client in formulating a Life Care Plan

Qualifications:

• Degree in social sciences, social work, nursing or a related field
• Knowledge and familiarity with the health care system
• Knowledge and familiarity with Medicare and insurance plans
• Experience with case management
• Outstanding communication skills, both written and verbal
• Excellent organizational and negotiation skills
• Ability to travel locally

Part time position with opportunity to be full time. Salary is commensurate with qualifications and experience.
