Provide mental health counseling support to organization’s Patient-Centered Medical Home goal of having healthier patients by instructing and coaching patients to a healthier way of life.

Responsibilities:

• Create and adapt wellness objectives to overcome barriers to good mental and physical health.
• Utilize standardized assessment tools to determine a patient’s ability to change or readiness for behavioral health intervention.
• Contact and perform interviews with patients who are identified as candidates who could benefit from behavioral health intervention and health coaching services.
• Encourage patients to express their feelings and discuss what is happening in their lives, helping them develop insight into themselves that may facilitate a deeper understanding of how having good mental health will support overall health.
• Assist primary care team in providing a safe and positive environment for patients to comfortably address potential behavioral health barriers in our chronic disease management programs.
• Help patients define personal goals and action plans.
• Run workshops and courses about mental health issues.
• Participate and collaborate with a patient’s personal primary care provider in joint visits and the development of an individualized care plan.
• Guide patients in the development of skills or strategies for dealing with their health problems including teaching life skills.
• Assist in efforts to coordinate patients with other healthcare services and community programs that address potential barriers to care.
• Develop and conduct group sessions or seminars to promote aspects of healthy living with patients and their families.
• Utilize face-to-face, telephone and telehealth opportunities to support patient efforts to healthier living or treatment barriers related to mental health issues.
• Learn about new developments in the field by reading professional literature, attending courses and seminars and establishing and maintaining contact with social service agencies.
• Counsel family members to assist them in understanding, dealing with, and supporting a patient’s treatment journey when necessary.
• Maintain regular appointment schedule to protect additional patient access to care opportunities.
• Maintain confidentiality in all facets of daily functions.
• Maintain a positive attitude toward patients and peers.
• Maintain accurate and complete documentation of therapy visits within the Electronic Medical Record
• Complete all tracking and reporting requirements for outcomes measurement.
• Advanced understanding of the Plans of Care adopted by our Patient-Centered Medical Home for Diabetes, Hypertension, Hyperlipidemia and our “At Risk” patient population which includes obesity and ER overutilization
• Support efforts to fully integrate mental health treatments in the MRHS Patient-Centered Medical Home structure.

Qualifications:

• High School diploma or equivalent
• Active license in the state of Nebraska (LMHP or LIMHP)
• Minimum 1year experience as a Mental Health Professional

Pay: $60,000.00 - $80,000.00 per year
