We are looking for a RN Case Manager to build relationships and support the wellness of our residents. In this role YOU will:

Build positive relationships throughout the home health team and community.
Work closely with patients, families and physicians to provide a personalized care experience by assessing individual needs.
Train others on new principles and methodologies of care to incorporate in clinical practices.
Maintain records and confidentiality regarding all aspects of a patient care.
Initiate and revise care plans, supervise and monitor CNAs and CMAs to meet the needs of patients.
Administer medications, maintain records and ensure care is provided to enhance the lives of our residents and meet state and federal guidelines.

Outstanding Candidates Will Have:

An active RN license in the state or hold a compact state license.
Current CPR Certification.
An active driver's license, acceptable driving record and access to reliable transportation.
Be able to independently lift 30lbs.
