The League of Human Dignity seeks full-time Services Coordinators to work from our Omaha office with adults with disability.  Services Coordinators assist people with disability to remain in their home and community by coordinating an array of services that supports the individual in becoming and remaining as independent as possible.  We are seeking a talented and highly motivated person who has a sincere desire to assist people.

As a Services Coordinator you will determine the eligibility of applicants for the Nebraska Medicaid Waiver Program for Adults with Disability, conduct a needs assessment, and in conjuction with Consumers develop a plan of services.  You would assist to recruit independent in home providers, negotiate rates, authorize and monitor services.  This is not an exhaustive description of job duties.

In orderto be considered for this position, you must have a Baccalaureate degree in relevant area of Human Services, and 2 years full-time experience in gerontology, rehabilitation, or community health fields are required.  Applicants must also be proficient in the use of computers and various computer software programs.  You will be expected to provide your own transportation, and we will reimburse you on a per-mile basis.  Qualified individuals with disability are strongly encouraged to apply.  These positions are open until filled.  EOE
