JOB DESCRIPTION

Job Title:    LPN

Department/Division:  Nursing

Reports To:      Director of Nursing

Status:  Non-Exempt

 

Shift:  2-10:30pm
          10-6:30am                                                                        
           Every Other Weekend and Holiday

General Summary:

To provide direct nursing care to the tenants of Bethany Heights Senior Living Community and to supervise the day-to-day nursing activities performed by nursing assistants/medication aids.  Such supervision must be in accordance with federal, state, and local standards, guidelines, and regulations that govern Bethany Heights Senior Living Community and as may be required by the Director of Nursing or Nurse Supervisor to ensure that the highest degree of quality care is maintained at all times.

Qualifications:

  • Must be an LPN licensed in the State of Iowa in good standing with no record of abuse.
  • Must be able to direct and supervise nursing care and maintain professional relationships with staff, tenants and families.
  • LPN’s must have supervisory course.
  • Must be able to speak and write the English language in an understandable manner.

Essential Job Functions:

I.    Administrative

  • Must be able to work every other weekend and Holiday
  • Accept and give tenant condition reports. Make rounds to evaluate status of tenants and determine needs.
  • Inspect environment, identify environmental problems, and initiate correction of problem.
  • Plan and delegate assignments.
  • Check for amount and condition of supplies, order supplies and equipment as necessary.
  • Communicates unit problems and needs to DON or ADON.
  • Gives feedback to DON or ADON regarding employee performance.
  • Consults with DON or ADON regarding staffing needs.
  • Consults with DON or ADON concerning the need for disciplinary action of employees, participates in disciplinary action as necessary and appropriate.
  • Identifies reports, documents incidents/accidents as necessary.
  • Supports and participates in Bethany quality assurance programs.
  • Assures that unit practices are consistent with regulatory requirements.
  • Participates on committees as necessary and appropriate.
  • Completes records and reports as necessary or required by DON.
  • Consistently attends scheduled meetings on time.
  • Complete consultant reports and contact doctors as needed.
  • Promote restorative and rehabilitation philosophy of maximum independence for tenants.
  • Promote infection control policies and advise ADON and DON of infections/contagious diseases.
  • Complete staffing schedules, billing, creating daily task sheets as assigned.
  • Communicating with transportation director to schedule medical Appointments as necessary for tenants.

II.    Clinical

  • Participates in tenant assessment and service planning activities, perform Nurse reviews as scheduled and as necessary, notify RN of any changes in Tenant condition.
  • Reviews, transcribes and implements physician’s orders.
  • Coordinates and participates in admissions, transfers and discharges of tenants.
  • Directs, supervises and assists staff in delivering nursing care.
  • Gives direct care as needed.
  • Communicates and coordinates care with tenants and their families.
  • Checks on nursing care activities of staff.
  • Counts narcotics.
  • Administers medications and treatments as required.
  • Implements, practices and assures staff compliance with infection control measures.
  • Prepares staff for tenant admissions.
  • Perform and document tenant 30 day post occupancy assessments, annual assessments and report to RN any condition changes appropriately.
  • Monitors effectiveness of medical regimen.
  • Perform physical assessment of tenant’s as directed.
  • Perform and document tenant 90 day nurse reviews.

III.    Educative

  • Provides instruction to staff as needed.
  • Participates in the orientation of new employees.
  • Assures staff attendance to in-service education programs and meetings
  • Attends continuing education programs to maintain competency and improve knowledge and skills.
  • Promotes a positive image of Bethany Heights Senior Living to staff, tenants and visitors.
  • Assures problems and complaints involving nursing services are managed and reported to RN in a timely and appropriate manner.
  • Protects privacy of tenants and staff.
  • Other duties as assigned.

Physical Requirements:

  • Must be able to move intermittently throughout the day.
  • Must be able to push, pull, move and or lift a minimum of 50 lbs. to a minimum distance of 3ft.
  • Must be able to assist in the evacuation of residents in an emergency situation.
  • Must be able to kneel, stoop or bend to assist residents.
