We are in search of experienced nurses that can serve on our Clinical Triage team and work collaboratively with other healthcare professionals to provide patient-centered quality care. Our triage nurses are the primary contact for all patients and are the voice for the provider’s entire clinical team. Each triage nurse is assigned to one physician/one team in order to give personalized care to the patients. They work with a multidisciplinary team to ensure that all inbound and outbound telephone calls are triaged in a manner such that appropriate levels of healthcare resources are deployed to each patient in a friendly and courteous manner.
Speaks on telephone with patients to assess symptoms, provide education and recommendations, and determine urgency of care needed
Collaborates with our physicians to determine what tests or services need to be ordered before the patient is seen by their primary care physician for both routine and acute visits, e.g., labs, imaging, clinical pharmacist, and behaviorist
Responsible for reviewing outside clinical data, e.g., labs, x-rays, and consultation reports and delegating to the proper team member
Coordinates patient specific care with nursing homes, home healthcare agencies, and hospitals
Provides education related to test results, lab work, new medications and treatment plan
Makes outgoing calls to patients to schedule follow-up appointments, labs, and imaging tests
Other work flow tasks, such as outside diagnostic results, orders requiring provider signatures, health record requests, and patient Web Portal questions

Skill, Knowledge, and Experience:
4 years of experience as a Licensed Practical Nurse (LPN) or Registered Nurse (RN). Experience in Internal Medicine working with adult patients, ideal
Experience performing phone triage in a clinical setting
Current license as a LPN or RN in the State of Nebraska
Telephone nurse experience
Strong proficiency with technology, i.e. computers
Skilled in electronic health record, ideally Allscripts and Microsoft Office applications
Clearly understands the limits of their education and experience, always practicing within their scope of education
