Mental Health Counselor CNA-17008628 

Provide care through effective planning, intervention and evaluation. Meet the needs of the patient, family, and multi-disciplinary team through effective communication, coordination, and documentation and under direct supervision of a registered nurse. Coordinate patient care and report patient changes to the appropriate team members. Ensure that the progress of patients is communicated to staff, physicians, other departments and family. Provide patient with assigned activities and tasks. Document patients� condition in medical and facility records according to policy and regulations. Provide therapeutic interventions on an individual and group basis which are congruent with team structure and program expectations. Participate in the development and implementation of treatment plans when requested. Maintain under the direction of the nurse, ongoing communication with internal and external individuals and/or agencies as appropriate, utilizing effective customer service skills. Ensure compliance with applicable regulatory guidelines and requirements. Demonstrate age related care skills as defined by unit patient populations.

Education/Experience:

Required: High School Diploma or GED, One Year Behavioral Counseling inpatient experience required or 3 years addiction experience (as evaluated by the supervisor). Successful completion of nurse assistant training program.

Preferred: College degree

Licensure/Certification:

Required: BLS on hire, Certified as a Nurse Aid in the State of Colorado

Knowledge and Skills:

  • Based on time in role and previous experience, caregiver applies the appropriate amount of reasoning, deduction and judgment in delivering quality patient care

  • Is able to handle the majority of instructions/directions provided without assistance of other caregivers

  • Is able to discern when to seek assistance in order to ensure quality patient care

  • Completes unit specific competencies/skills by the end of orientation, demonstrates on-going competencies and skills and attends annual mandatory education of such

  • Unit specific competencies and skills to include the following:

    Admits except for assessments required by RN, co-signed by RN

    Discharge notes and paperwork, co-signed by RN

    Vital signs

    Attend annually required mandatory education and skills fair

    Develop skills around basic addiction interventions, such as motivational interviewing and supportive

    counseling
