The Care Coordinator/Social Worker, LCSW is responsible for patient care coordination, social work services and discharge planning in the hospital and in the Emergency Department.  He/she works with other social workers and R.N. Care Coordinators within a team model to coordinate patient care and discharge plans.  Ongoing patient needs are discussed in the daily Multidisciplinary Patient Care Planning Meeting (PCPM), which is facilitated by the Social Work Care Coordinators.  Assess all œHigh Risk patients and follows up on any Advance Directive needs indicated by Admissions. Provides crisis intervention and short term counseling services.  The Care Coordinator/Social Worker will be responsible for mental health evaluations and placing patients on a 72-hour Mental Health hold when needed, and determining that patient's discharge.   Is knowledgeable of community resources, referrals and acts as community liaison.  Is responsible for patient chart documentation, confidentiality and the use of œMedical Release of Information when appropriate.  Maintains performance improvement activities within the Department and participants in quality assurance activities.  Assures patient satisfaction through communication. To be willing to work weekdays, weekends and on-call.  He/she will have limited supervision.  Updates policies and procedures and Care Coordination forms.  Performs all other duties as assigned. 

QUALIFICATIONS
  • Education:  LCSW with a Masters degree in Social Work
  • Experience:  One year experience in hospital social work or one year experience in a social agency or community organization dealing with health and/or welfare preferred.
  • Knowledge, Skills and/or Abilities:  Knowledge of the principles and practices of hospital social work  and counseling.  Knowledge of community, social, health and welfare programs.  Ability to coordinate effectively and appropriately. Must have excellent communication skills. Ability to collaborate and cooperate with other disciplines.  Excellent negotiation and conflict management skills.   Ability to competently organize, facilitate, educate, document, and work effectively with patients, families, physicians, managers, administrators, community contacts, and other disciplines.  Must consistently use good judgment in dealing with all internal and external customers.  Must serve as a positive community liaison/patient representative on behalf of PVMC.  Promotion of performance improvement goals.  Knowledge of Medicare, Medicaid, and TJC guidelines preferred.  Computer skills.  Bilingual preferred.  This job may require the employee to be responsible for the assessment, treatment, or care of patients of all ages, including neonate, child, adolescent, and geriatric.  Consequently, the employee must be competent in the interpretation of a patients self-report or behavior, and this information must be interpreted with an understanding of the cognitive, physical, emotional and chronological maturation process.  The treatment of patients should be individualized and should reflect an understanding of the developmental needs and range of treatment needs for each patient.  (See œCompetency Assessment policy)
