Opportunity Description:

 

SCL Health is seeking a Hospice and Palliative Care Nurse Practitioner to provide direct and indirect care to patients and families experiencing chronic and/or terminal illness. Our goal is to enhance the quality of life of the patient. This position will flex between our Hospice and Palliative programs.

  • Work collaboratively with an amazing interdisciplinary team of Physicians, other Advanced Practitioners, Social Workers and Chaplains
  • Hours are Mon - Fri 8am - 5pm
  • Call is 1 night per week on-call and every 5th weekend in the Hospital Inpatient Unit
  • Palliative Care offered to patients in the clinic setting, patient home and/or hospital setting
  • Responsible for program development and education.
  • Function independently to perform physical assessments of Palliative and Hospice patients.

Qualifications:

  • Completion of a postgraduate Nurse Practitioner Program, required
  • Valid license/certificate as a Nurse Practitioner in the State of practice, required
  • DEA and full prescriptive authority in the state of CO, required
  • Current Basic Life Support Certification, required
  • Minimum 5 years experience as a Registered Nurse with at least one year in a geriatric setting, highly desired
  • Minimum 2 years experience as a Nurse Practitioner, required
  • One year of Hospice/Palliative Care experience, highly desired
