Hillcrest Home Care, a healthcare provider focused on aging adults, has opportunities for a Quality Assurance Performance Improvement (QAPI) Specialist candidate interested in performing on-going chart audits to identify opportunities for improvement. This will be a Monday-Friday 8am-5pm position with No On-Call and No Weekend shifts.  This a great advancement opportunity for an experienced Home Care RN looking to advance to the next level in Home Health with no hands-on clinical care.

The QAPI Specialist is a key role that supports our organization and the success of our Home Health nursing and therapy team members.

Responsibilities:

  • Assist in the completion of audits performed by the QAPI department and prepare reports as needed for QAPI meetings
  • Perform ongoing chart audits to identify opportunities for improvement
  • Participate in the development and implementation of appropriate plans of action to correct identified deficiencies as needed
  • Assist with the State Survey process for Home Care and Hospice in accordance with survey standards
  • Assist in the development and delivery of educational programs as it pertains to QAPI and Homecare services
  • Assign appropriate diagnosis codes to all clients serviced

Requirements:

  • Qualified applicants must have a minimum of 2 years' experience with a skilled home health organization
  • Bachelor's degree from an Accredited program of Nursing
  • At least 2 years of OASIS Assessment experience
  • Previous experience in Quality Improvement functions preferred
  • Detailed knowledge of assessments, clinical documentation, and home health agency regulations
