Hillcrest Health Services, Nebraska's premier continuum provider of health care services for aging adults, is growing.  We are currently the Omaha Area's largest continuum of healthcare services for that aging adult, and we a looking for a leader to help us grow to the next step. 

Hillcrest Home Care is actively seeking a knowledgeable and experienced Clinical Care Coordinator (CCC) candidate with a history of management in a home care environment. This position is responsible for the direct coordination of the client care processes in the home care setting, and ensuring an interdisciplinary team approach to quality home health services to clients to assist them in meeting their maximum level of independence.

Key Responsibilities:

  • Organizing, developing and directing the overall operation of services in accordance with current federal, state and local standards, guidelines and regulations. 
  • Responsible for clinical program development, implementation, monitoring and accountability process that result in high quality care within assigned budget.
  • Ensures staffing needs are met through effective hiring, onboarding, and retention of team members.
  • Maintain a high customer satisfaction rating
  • Responsible for creating innovative and successful services/programs for the agency which spur growth in revenue and market share.

Key Requirements:

  • Must be a licensed RN in the State of Nebraska or compact state
  • BSN preferred
  • Highly skilled in communication, critical thinking and organization
  • The ideal candidate will have a minimum of two years' experience with home health (leadership experience preferred) 
  • Detailed knowledge of assessment process, billing procedures, clinical documentation  procedures and home health agency regulations
