Clinical: Minimum Data Set (MDS) / Assessment Coordinator

RIDGECREST REHABILITIATION CENTER:  Innovative corporate culture and opportunities for growth. Our skilled unit, long-term nursing care section, and dementia unit's residents are well suited for group living and are congenial.  The facility is located one block from a metro bus stop.

POSITION: Immediate opening at Ridgecrest for a Full Time Minimum Data Set (MDS) / Assessment Coordinator Nurse. It is required that applicants have at least three years experience with elders in a skilled nursing home or long-term care facility.  It is preferred that the MDS Coordinator has two or more years experience in long-term care, ICD 10 experience is desired.  Qualified candidate must be is a registered nurse with recent MDS experience.

MANAGEMENT RESPONSIBILITIES: : Plan, supervise, and coordinate the assessment of nursing home residents. Monitor policy, procedures, and protocols to fulfill individualized care plan processes. Support the highest level of safety, physical health, and mental well-being of our facility residents.

SERVICE JOB DUTIES: Complete the MDS 3.0 reporting in accordance with State and Federal regulations. Provide census and payor type reports to meet the facility and regulatory agencies’ required standards. Actively implement the plan of care and provide medication management so that the individualized services are accomplished accurately for new and current residents.

REQUIREMENTS - A) Current, unencumbered license to practice in Nebraska.  B) Nursing Degree from an accredited nursing program, college, or university. C) CPR Certification. D) All facility staff must consent to have an initial and possible random drug screens.  E) The ability to push / pull at least 25 - 35 lbs.
