Department/Position Details:

  • Maintain a manual of approved medical record policies, procedures, abbreviations and forms that govern the content, documentation and formats for the medical records of the hospital's active and discharged clients; coordinate the process for the review and approval of additions/changes to the manual.
  • Disseminate information and educate appropriate hospital staff about the hospital's policies, procedures and expectations regarding medical records. 
  • Store, safeguard and provide access to the medical records of discharged clients in accordance with applicable laws, rules, policies, procedures and JCAHO standards.
  • Conduct ongoing reviews/audits of the hospital's medical records. Analyze health records for accuracy, completeness and quality.
  • Oversight of all medical coding related activities including diagnoses and medical procedures for billing.
  • Oversight of all medical transcription services to all hospital physicians and to other professional staff as needed.
  • Provide support for the integrity of the information in the hospital EMR including compiling reports of analyses of data entry errors for performance improvement.
  • Create and facilitate education related to HIM updates and best practices.
  • Serves as a resource for department managers, staff, physicians, and administration to obtain information or clarification on health information management topics.
  • Comply with applicable laws, rules, policies, procedures and JCAHO standards governing the confidentiality, security and integrity of the medical record and promote compliance hospital-wide.
  • Establishes, implements, and maintains a formalized review process for HIM compliance. Conducts regular audits and coordinates ongoing monitoring of documentation adequacy.
  • Provides all reporting for internal and external stakeholders in meaningful format. Serve as the subject matter expert on modeling and analyzing patient data.
  • Designs and maintains Data Analytics environment pertinent to HIM functions.
  • Identifies variations in data capture and management.
  • Works with Informatics Group to ensure proper education of staff in all systems.
  • Establishes, implements, and maintains processes for monitoring, evaluating, and improving coding accuracy, timeliness, and compliance.
  • Serves as information privacy consultant to the organization for all departments and appropriate entities.
  • Serve as a subject matter expert for all HIM related activities.
  • Comply with safety activities, policies and procedures and regulatory requirements such as OSHA and The Joint Commission.
  • Employee is responsible for all other duties as assigned for which competency has been demonstrated.                                             

 

Position Requirements:

Education:

  • Bachelor’s Degree in health information management or related field required.
  • Formal HIM education with accreditation or certification (RHIA, RHIT, or CCS) at the national level.

Experience:

  • Four years’ experience working in a health information management department preferred.
  • Three years in a management capacity preferred.

Physical:

  • This position is classified as Light Work in the Dictionary of Occupational Titles, requiring the exertion of up to 20 pounds of force frequently, and/or a negligible amount of force constantly (66% of the time) to move objects. Physical Demand requirements are in excess of those for Sedentary Work.
