Job Description
We are currently looking for a Resource Specialist who will interview and guide pediatric patients with behavioral and/or mental health conditions and their families. Working as a member of the pediatric Integrated Health Services team, the Resource Specialist will plan and coordinate care based on needs; evaluate the effectiveness of the care provided; and identify and evaluate the significance of social, environmental, and emotional factors relative to behavioral and/or mental health conditions. He or she will use the principles of family-driven, community-based, comprehensive, and culturally competent care in the provision of services for children with behavioral and/or mental health care needs. The Resource Specialist will lead Family Team meetings to develop a care plan containing needs, strengths, and goals for treatment.
The ideal candidate has a Bachelor's degree in a Human Services field and at least three years of experience in Social Work or a related field. He or she must have knowledge of and experience with community-based and family-centered child health resources and experience in care coordination. RESPONSIBILITIES

- Follows the principles of family-driven, community-based, comprehensive, and culturally competent care to provide services for children with behavioral and/or mental health care needs
- Leads Family Team meetings to develop a care plan containing needs, strengths, and goals for treatment
- Performs other duties as assigned
QUALIFICATIONS
- Bachelor's degree in a Human Services field required
- At least three years of experience in Social Work or a related field required
- Must have experience in care coordination
- Must have knowledge and experience with community-based and family-centered child health resources
- Excellent written and verbal communication skills required
- Excellent interpersonal skills
- Basic computer skills required, which includes turning on a laptop and using email, Word, Excel, electronic health record (EHR) systems, portals, etc.
