Interview and guide pediatric patients with behavioral and/or mental health conditions and their families, as a member of the pediatric Integrated Health Services team. Plan and coordinate care based on needs, evaluate effectiveness of care provided; identify and evaluate the significance of social, environmental and emotional factors relative to behavioral and/or mental health conditions. Utilize the principles of family-driven, community-based, comprehensive, and culturally competent care in the provision of services for children with behavioral and/or mental health care needs. Lead Family Team meetings to develop a care plan containing needs, strengths, and goals for treatment.
QUALIFICATIONS:
Bachelor's degree in a human services field is required
Minimum 3 years experience in Social Work or related field is required.
Excellent written and verbal communication skills are required.
Must have competency in computer basics which includes turning on a laptop, using email, Word, Excel, electronic health record (EHR) systems, portals, etc.
Experience in care coordination
