The Open Door Mission is a gospel rescue mission providing basic needs and life-changing programs for the homeless and needy.
In this position you will insure that the Gospel is shared with all men, provide appropriate healthcare management and coordinate care for adult males with mild to moderate and complex illnesses. You will be responsible to oversee programs, monitor the individual’s overall function within men’s ministries and implement the objective to equip them with the tools to be self-sufficient and independent.
Essential Duties and Responsibilities:
Provide healthcare & social services case management for guests and develop their short and long term goals. Conduct assessments including gathering information and formulating assessment of the families current needs, life situation and resources. Develop an Individual program plan for each program member based on needs.
The ability to advocate for our guests by working with other healthcare/social services professionals that are volunteers here at ODM or are in an established practice within the area.
Establish and maintain documentation, files, notes, regarding guest’s progress. Refer and network ODM guest to outside agencies as necessary to ensure client services. Insure each guest receives 45 minutes of case management. Provide accountability.
Provide emergency crisis counseling, problem solving, de-escalation strategies to staff and program members. Represent the ODM through community functions, to volunteers and through tours of the ODM. Greet guest/volunteers and facilitate their time at GTMC/ODM as necessary.
Develop policies and procedures for operation of the department.
Keep developing techniques, skills and intervention that is effective in changing the thinking process of the homeless, chemical dependent, and mentally ill.
Administer drug/alcohol tests. Oversee documentation of these tests.
Knowledge, Skills, and Abilities: LPN/RN required, BSN preferred.
Minimum one year nursing experience required. Knowledge of human services, local human service agencies, homelessness, crisis intervention, chemical dependency and recovery, mental illness, domestic violence, social issues, evangelism, case management, scheduling, public relations, and counseling. Open Door Mission will ensure appropriate knowledge is acquired to help you succeed as a case manager.

Mature person who is pursing Jesus, giving of themselves sacrificially to others, and is being spiritually fed outside of Open Door Mission. She/he is eager to learn about the needs of our guests and the resources to assist them. She/he is extremely flexible and able to work with the homeless and other staff members under a variety of stressful situations.
