We are looking for a Service Coordinator and a member to join our amazing team!!

This position will combine many different aspects in the home care spectrum and success will be measured in effectiveness in developing plans for our clients satisfaction along with work with referral sources.

Essential Functions:

  • Takes accurate referral/service request information by phone. Determines type and duration of services necessary to meet client needs. Quotes bill rate based on service provided and explains billing process.
  • Performs insurance verification utilizing information collected on service request and obtains billing/coverage information for private insurance cases.
  • Communicates new assignments to care coordinator and scheduler.
  • Processes data necessary to establish client service, client files, and initiate invoicing.
  • Contacts clients to set up appointments for service delivery.
  • Maintains statistics of weekly/monthly referrals and their sources.
  • Follow up and follow clients thru care.
  • Participates in on-call rotation as assigned.

Scheduling Functions:

  • Develops monthly plans by matching caregiver qualifications and availability to client’s needs.
  • Communicates new assignments and/or schedule changes to caregivers and clients.
  • Processes either manually or via computer, the data necessary to initiate accurate payroll and billing processes.
  • Participates in on-call rotation as assigned. Participates in client case conferences as requested by Scheduler.
  • May assist with the input, verification, and release of billing and payroll information as well as the assembly of data for financial reporting purposes.
  • Computes wages and records data for use in payroll processing and competitive rate studies.
  • Works with the Assistant Coordinator in conjunction with the Team to assist in resolution of caregiver issues.
