SUMMARY:

Responsible for connecting youth to supports and services at the point of entry into PEO through the Connected Youth Initiative (CYI) core component of central navigation.  Ensures that services and supports are focused on the seven target areas for youth transitioning into adulthood:  Education, Employment, Housing, Finances, Transportation, Mental/Physical Health, and Permanency. 

Responsible for the creation of the Project Everlast Pathway Plan for each youth that enters PEO.  Assists with the planning/implementation of other PEO activities.

ESSENTIAL DUTIES AND RESPONSIBILITIES: 

Coordinates efforts between Youth Engagement Specialist, community service providers/partners, Community Response/Bring Up Nebraska collaboratives in the Omaha metro area, and the youth to create Pathways Plans that contain the most appropriate supports and services available in a timely manner.

  • Works directly with the Youth Engagement Specialist to develop, conduct, and implement Central Access consultations, ensure the Participant Information Form is completed and entered into the database, and refer for any additional independent living evaluations deemed necessary,
  • Conducts regular/periodic PEO orientation sessions. 
  • Works with youth and providers to ensure youth-friendly intake procedures when accessing services and supports.
  • Create and maintain relationships with Project Everlast Omaha program partners and community partners to provide the best coordination of services for youth.
  • Coordinates with Douglas County Community Response (DCCR) and Lift Up Sarpy County to ensure a streamlined central navigation system and process for young adults and/or expectant and parenting young adults and their families.
  • Develops action steps to track youth outcomes and re-engage youth who have lost contact with Project Everlast and community providers.  
  • Work to identify supports and resources that can assist current and former foster youth.
  • Assists in the planning and facilitation of PEO Youth Activities.
  • Attend Project Everlast, Partner Programs, and Community meetings as needed.
  • Participate in statewide Bring Up Nebraska and Connected Youth Initiative meetings and workgroups as needed or requested.
  • Recruit and interview social work practicum students.  Attend any required/necessary meetings and recruitment fairs. 
  • Develop and maintain database reporting and functionality in partnership with PEO Staff and partners.
  • Assist with the development, outreach and coordination of housing, transportation, and maternity programs.
  • Regular attendance.

Other duties may be assigned.

SUPERVISORY RESPONSIBILITIES: This position currently has no supervisory responsibilities.

WORK DIRECTION RECEIVED:  Works under direct supervision

SCOPE/NATURE OF DISCRETION:  Delegated discretion; authority to initiate and carry out actions that are deemed essential.

QUALIFICATIONS:

EDUCATION AND EXPERIENCE:

Required: 

  • Associates degree in related field and
  • 3-5 years’ experience in working with the child welfare system, and
  • Demonstrate extensive knowledge relating to supports and services available for youth aging out of the foster care system, and/or
  • Lived experience in these systems of care.
  • Any equivalent combination of experience, training, and/or education will be considered.
  • Valid Driver’s License and proof of insurance.

Knowledge

Abilities:

  • Establish and maintain effective working relationships with partner organizations
  • Gather, analyze, and review collected information and make recommendations
  • Stay calm during intense situations and work under pressure
  • Facilitate meetings to gain input and disseminate information

 

Preferred: 

  • Bachelor’s degree in social work, human service, public administration, education, criminal justice, or other similar degree.

LANGUAGE SKILLS:

  • Ability to read, analyze and interpret documents such as grant solicitations or governmental regulations. 
  • Ability to write grant proposals and progress reports as well as routine reports and correspondence.  
  • Ability to respond to common inquiries about the organization from the public and/or business community. 
  • Ability to effectively present information to public groups, top management, and/or boards of directors.

 

MATHEMATICAL SKILLS:

  • Ability to add, subtract, multiply, and divide in all units of measure, using whole numbers, common fractions, and decimals. 
  • Ability to compute, rate, ratio, and percent and to draw and interpret bar graphs.  Ability to read and interpret demographic and statistical research data.

 

REASONING ABILITY:

  • Ability to solve practical problems and deal with a variety of variables in unique situations.
  • Ability to interpret a variety of instructions furnished in written, oral, diagram, or schedule form. 
  • Ability to define problems, collect data, establish facts, and draw valid conclusions. 
  • Ability to interpret statistical research findings (such as Census data) and to interpret mathematical data accurately in written communications. 
  • Ability to plan, schedule, and manage multiple activities at once.

 

PHYSICAL DEMANDS:

  • While performing the duties of this job, the employee is regularly required to sit and talk or hear. 
  • The employee frequently is required to use hands to finger, handle or feel.
  • The employee is occasionally required to stand, walk, and reach with hands and arms.
  • Ability to drive

 

WORKING CONDITIONS:

  • Flexible schedule including evenings and weekends. 
  • Willingness to occasionally travel within the state.
