POSITION SUMMARY

The goal of the Health Navigator is to help enhance access to high-quality medical services in order to improve the overall health status and lower the cost of care for the community that CDHC serves. As part of the population health team, the Navigator will focus on quality of care metric dashboards to help identify and close gaps in care. The Navigator will accomplish this goal by identifying patients overdue for healthcare services and supporting efforts to ensure completion of those services. The Navigator will work with the Population Health Manager and other members of the population health team to review current approaches and develop data-informed improvement strategies. The Navigator will also provide education related to health services to CDHC patients in an effort to raise awareness and improve access to services.

POSITION-SPECIFIC DUTIES & RESPONSIBILITIES

  • Understands and is adept in using population health tools within CDHC EHR as well as from payors and grant funders
  • Reviews reports generated from population health applications (including EHR) to identify patients due for specific medical services and screenings
  • Contacts patients due for specific medical services and screenings and assists them with making necessary appointments
  • Coordinates with clinical team to assist patients in the completion of screening services by providing follow-up and navigation support.
  • Obtains clinical data from external organizations, enters data into EHR, distributes data to providers as well as patients, when necessary
  • Assists in patient recruitment for screening and health services events
  • Acts as a patient liaison by ensuring patients connect with the Patient Financial Service team, assisting patients in making appointments for specialty services, and providing educational support
  • Collaborates with outside agencies to obtain specialty services for our patients where necessary
  • Assists with staff education and training as appropriate and necessary
  • Performs other duties as assigned.

EXPECTATIONS

In the first year, demonstrate significant progress in the following areas:

  • Increase performance rates in quality metric dashboards.
  • Work with population health team and clinical staff in establishing plan to meet monthly and annual performance
  • Develop effective working relationship with provider staff

POSITION REQUIREMENTS

  • Language: English fluency
  • Work Hours: Monday –Friday 8:30am – 5:30pm; flexibility to adjust to meet patients during clinic operation hours
  • Travel: Minimal
  • Exposure: The exposure characteristics described here are representative of those an employee encounters while performing the essential functions of this position. For example, while performing the essential functions of this job, the employee seldom is exposed to fumes or airborne particles and toxic or caustic chemicals. Additionally, the employee may occasionally be exposed to blood borne and other hazardous chemicals. Finally, the noise level in the work environment can seldom be quite loud. In all cases personal protective equipment will be provided to the employee in combination with adequate ventilation and other engineering controls to minimize the risk of exposure or other hazardous occurrence
  • Education: High School Diploma or equivalency required. Bachelors in Human Services field Preferred but not
  • Experience: Patient/client navigation services preferred
  • Expertise: Strong interpersonal skills
  • Proficient verbal, written and reporting skills
  • Microsoft applications (experience with Excel required), internet, email Good record keeping methods
  • Ability to achieve measureable outcomes Strong organizational skills
  • Ability to handle multiple tasks Knowledge of medical terminology
