POSITION SUMMARY

The OHS Outreach & Health Education and Training Coordinator is responsible for outreach coordination, intake and enrollment, and Health Education and Training activities. This responsibility includes ensuring effective collaboration, implementation, maintenance and total quality coordination of all OHS outreach activities, health education and training activities, and printed outreach materials, educational materials, and curriculums. In addition, assist in the preparation of documentation that reflects program performance measures, monthly participant data, monthly program data and all other program goals and objectives as outlined by designated funding sources.

POSITION-SPECIFIC DUTIES & RESPONSIBILITIES:

  • Must be able to work independently with minimal supervision utilizing independent judgment when executing daily activities impacting the OHS program;
  • Use personal knowledge of the community and the OHS target area to identify strategies and opportunities with health and social service organizations, churches, businesses, schools, and community events for the purpose of reaching at-risk pregnant women and women of child bearing age for participation in the OHS program;
  • In collaboration with all members of the OHS staff, and under the direction of the Project Manager, lead and implement an annual outreach plan that identifies a variety of community locations and opportunities with health and social service organizations, churches, businesses, and community events for the purpose of participant recruitment in the East/Northeast project area;
  • Maintain a department schedule of all outreach activities and contacts
  • Collect, track, and report, on outreach and HET data and assist with the completion of monthly reports and the evaluation of outreach and HET goals and objectives;
  • Ensure consistent and reliable documentation of activities in compliance with all organization and department standards;
  • Responsible for the ongoing assessment of program participant needs in relation to their meaningful engagement in the OHS program;
  • Track and report on progress towards meeting OHS participant retention goals;
  • Attend OHS CAN meetings representing the outreach component as needed;
  • Represent OHS at designated workshops, health fairs, conferences, etc.
  • Coordinate the use of OHS volunteers in appropriate outreach activities;
  • Attend identified community meetings and participate on designated committees for the purpose of ensuring on-going outreach and health education and training collaborations;
  • Responsible for coordinating health education and training for OHS staff, program and community participants, health care and MCH providers serving families in the OHS project area;
  • Responsible for tracking and reporting health education and training for OHS staff, program and community participants, health care and MCH providers serving families in the OHS project area;
  • Responsible for monitoring and tracking continuing education and certifications for all OHS staff;
  • Ensure that all printed OHS educational materials and curriculums are appropriately reviewed, tracked and replenished;
  • Assist in fund development activities to support the continued funding of OHS incentive programs;
  • Actively seek out funding opportunities to increase the sustainability of the health education and training component;
  • Oversight and supervision of Patient Resource Coordinator and contract referrals
  • Other duties as assigned.

POSITION REQUIREMENTS:

  • Education:Bachelor’s Degree and 3-5 years of maternal and child case management experience required
  • Licensure: Current Nebraska License
  • Certification: HIV/AIDS Counselor and Tester; Adult and Infant CPR;
  • Experience: Two plus years clinical experience or public health experience in working with and advocating for families residing in East/North East Omaha; experience in working with diverse ethnic and racial populations; and demonstrate effective customer service skills; Demonstrate case management and supervisory experience, experience working with diverse racial service/treatment plans;
  • Expertise: Excellent oral communication skills, knowledge of available community based health and social services; self-starter, knowledge of project target area; strong organizational skills; multi-tasked and flexible.
  • Language: English as a primary language, Spanish speaking helpful
  • Hours of Work: 8:00 a.m. to 4:30 p.m., some evenings and weekends
  • Travel: Must be willing to travel in and out of state as needed; must possess valid State of Nebraska driver’s license and own mode of transportation.
  • Exposure: The exposure characteristics described here are representative of those an employee encounters while performing the essential functions of this position. For example, while performing the essential functions of this job, the employee occasionally is exposed to fumes or airborne particles and toxic or caustic chemicals. Additionally, the employee may occasionally be exposed to blood borne and other hazardous chemicals. Finally, the noise level in the work environment can occasionally be quite loud. In all cases personal protective equipment will be provided to the employee in combination with adequate ventilation and other engineering controls to minimize the risk of exposure or other hazardous occurrence.
