Job Description


The Mental Health Advocate is responsible for community organizing and advocacy involving individuals with mental health conditions within the CDHC mental health system. The Community Advocate provides leadership in facilitating community-based support with an emphasis on supporting the Behavioral Health Department. This position is a part of the primary care treatment team as well as a part of the Behavioral Health Team. The Mental Health Advocate will welcome, inspire and provide hope within a safe environment since “every door is the right door”. The Mental Health Advocate identifies triages and manages patients with behavioral health problems. In addition, the position will provide quality mental health interventions for diverse populations. The Mental Health Advocate will develop specific behavioral change plans for patients for target populations. In addition, have the ability and flexibility to treat patients across the CDHC enterprise regardless of age, economic status, gender, gender choice, ethnicity and or race.


  • Understand CDHC Policies and Procedures
  • Assist BH provides with assistance in helping patients meet their individual needs
  • Has a solid understanding of 206 regulations
  • Assist primary health care providers in recognizing and treating mental disorders and psychosocial problems.
  • Assess the clinical status of patients referred
  • Completes clinical documentation according to policies and procedures (CDHC and 206 regulations)
  • Work with primary care team to treat and manage patients with chronic emotional and/or mental health problems
  • Assist in the detection of “at risk” patients and the development of relapse plans to prevent further psychological or physical complications
  • Attend and participate in required meeting and activities
  • Serve as a trustworthy member of the Behavioral Health Team
  • Abide by State and Federal guidelines as it pertains to code of ethics, code of conduct, billing submission and therapeutic relationships
  • Work diligently and professionally with internal and external partners
  • Assist with the integrated model and conduct Warm Hand Off’s at any of our sites
  • Assist in implementing the EPIC program as indicated by the Adolescent Health Program
  • Gain 3000 indirect and non-direct hours to meet the requirements in obtaining full licensure as a LMHP and or PLMHP
  • Represent CDHC in the community and at public meetings about issues affecting people with mental health conditions
  • Provide supportive drop in services to CDHC Behavioral Health Department
  • Serve as a positive healthy advocate for CDHC and the community
  • Coordinate outreach, advocacy and education activities for relevant issues
  • Attend events relevant to outreach and accompany the CDHC Outreach Team
  • Track outcomes and provide reports
  • Use the Electronic Health Record when charting on patients
  • Serve as a liaison between the BH CDHC and community
  • Potentially provide in-home care to those approved patients

  • Comfortable with use of Windows operating system ( Word, Excel, Power Point and Outlook)

  • Education: Master’s Degree
  • Licensure: Provisional Licensed as a Mental Health Therapist in the State of Nebraska (PLMHP)
  • Experience: 4 plus years of experience working with youth, adults and or families
  • Expertise: Working knowledge of evidence-based treatments for mental health conditions. Ability to make quick and accurate clinical assessments of mental and behavioral conditions. Ability to work through brief patient contacts, including via telephone and teleconferencing. Ability to train and teach effective behavioral health interventions. Ability to work effectively as a team, interfacing with patients, primary care providers, consulting psychiatrist and mental health specialists, as well as with administrative and support staff. Ability to effectively represent CDHC’s interests in the community and maintain effective working relationships among co-workers, public, private, and professional groups. Ability to read, analyze, and interpret general business periodicals, professional journals, technical procedures, or governmental regulations.
  • Language: English – other languages a bonus
  • Work Hours: Combination of Traditional and Non- Traditional Hours (Weekend and late evenings)
  • Travel: Locally, intrastate, and interstate, as necessary
  • Exposure: The exposure characteristics described here are representative of those an employee encounters while performing the essential functions of this position. For example, while performing the essential functions of this job, the employee is rarely exposed to fumes or airborne particles and toxic or caustic chemicals. Additionally, the employee may occasionally be exposed to blood borne and other hazardous chemicals. Finally, the noise level in the work environment can occasionally be quite loud. In all cases personal protective equipment will be provided to the employee in combination with adequate ventilation and other engineering controls to minimize the risk of exposure or other hazardous occurrence.
